摘　　要：目的探讨肾上腺原发性神经内分泌癌(NEC)的CT表现。方法选取病理证实的3例肾上腺神经内分泌癌(排除转移为原发)患者,观察肿瘤的数量、位置、外形、大小、密度、增强扫描的强化程度、与周围组织的关系、有无腹膜后淋巴结肿大。结果3例患者4个肿瘤;2例位于右侧肾上腺、为单发,1例为双侧肾上腺各1个肿瘤;肿瘤径线大小范围2.3 cm×2.6 cm~4.8 cm×6.1 cm;2例3个肿瘤形态规整,呈椭圆形,边缘清晰,密度均匀;1例肿瘤CT表现为出血并肿瘤破裂,增强扫描无明显强化,1例肿瘤轻度较均匀强化,1例2个肿瘤呈轻度延迟性均匀强化;3例肿瘤均未见坏死、囊变、钙化;均无腹膜后淋巴结肿大。结论肾上腺原发性NEC的CT表现具有一定特征,对于鉴别诊断肾上腺癌有临床意义。Objective To explore the CT findings of primary adrenal neuroendocrine carcinoma(NEC).Methods CT images of 3 cases of adrenal NEC(metastasis was excluded)proved by pathology were retrospectively reviewed.The tumors’number,site,shape,size,density,the degree of enhancement,relationship with surrounding tissues and enlarged retroperitoneal lymph nodes were analyzed.Results There were 4 lesions in 3 cases.2 cases located on the right adrenal gland and showed single lesion,1 case showed one tumor on each side of the adrenal gland.The diameters of the tumors were 2.3 cm×2.6 cm~4.8 cm×6.1 cm.3 tumors of 2 cases had regular oval shapes and well-defined margines,showed uniform mass density.1 case presented as bleeding and rupture,without obvious enhancement,1 case showed more uniform mild enhancement,2 tumors of one case showed mild uniform delayed enhancement.No necrotic cysts or calcification were observed in 3 cases;There were no enlarged retroperitoneal lymph nodes in all cases.Conclusion The CT imaging presentations of primary adrenal neuroendocrine carcinoma have some certain speciticity,which has clinial signiticance for differential diagnosis of adrenal carcinoma.
