摘　　要：目的观察复方青黛灌肠液保留灌肠快速缓解溃疡性结肠炎(UC)炎症活动的疗效。方法本研究采用治疗前后自身对照研究方法,对纳入的42例UC患者采用传统直肠给药的方法予单纯复方青黛灌肠液保留灌肠治疗,比较患者灌肠治疗14天前后主要临床肠道症状、黏膜内镜及病理改变、实验室指标、生活质量评分及不良反应情况。结果复方青黛灌肠液保留灌肠治疗14天后,临床有效率为92.86%(39/42),临床缓解率为59.52%(25/42)。内镜黏膜愈合率为72.97%(27/37),组织学缓解率为29.73%(11/37)。与治疗前比较,治疗后患者各临床肠道症状评分、Mayo内镜评分(MES)分值、UC内镜下严重度指数(UCEIS)评分、组织病理学Geboes指数、粪便钙卫蛋白(FC)与大便隐血试验(OB)下降(P<0.01),治疗后UCEIS评分远端结直肠较近端结肠改善明显(P<0.01),血红蛋白(HGB)与白蛋白(ALB)、生活质量评分升高(P<0.05)。结论复方青黛灌肠液可于14天内快速缓解UC炎症活动,并改善患者生活质量。Objective To observe the effect Compound Qingdai Enema rapidly alleviates the inflammatory activity in patients with ulcerative colitis(UC).Methods A pre-and post-treatment trail was self-controlled used in this study.Totally 42 patients with active UC were treated with Compound Qingdai Enema by traditional rectal administration method.The main clinical symptoms,mucosal endoscopic manifestations,mucosal histopathology,laboratory indicators,quality of life scores and adverse reaction evaluation were observed and compared before after 14 days treatment.Results After 14-days treatment,the clinical effective rate was 92.86%(39/42),the clinical remission rate was 59.52%(25/42),the endoscopic mucosal healing rate was 72.97%(27/37),the histological remission rate was 29.73%(11/37).Compared with baseline,each clinical intestinal symptom score,Mayo Endoscopy Subscores(MES)score,Endoscopic Severity Index of Ulcerative Colitis(UCEIS)score,Geboes index,laboratory indicators such as fecal calprotectin(FC)and occult blood(OB)score decreased after treatment(P<0.01).The improvement of UCEIS score in distal colorectum was more obvious than that of proximal colon after treatment(P<0.01),occult blood(HGB),albumin(ALB),and quality-of-life evaluation increased after treatment(P<0.05).Conclusion Compound Qingdai Enema can rapidly relieve UC inflammatory activity within 14 days and improve the patients'quality of life.
