从“阴阳反作”探讨“升降失常”类疾病诊治新思路

摘　　要：[目的]基于“阴阳反作”理论,探讨中医疾病中“升降失常”类疾病的病机及其对应治疗方法。[方法]通过总结“阴阳反作”中阴阳所对应的概念,就阴阳间的三种关系对“阴阳反作”理论进行进一步探讨,并分析“升降失常”类疾病的病因病机及其对应的治疗方法。[结果]根据疾病表现,“升降失常”类疾病可以分为“阴之反作”“阳之反作”及“阴阳反作”三类,其中中风、2型糖尿病、糖尿病肾病、慢性肾脏病为“升降失常”类疾病的典型例子。在治疗上,这些疾病均可以调节升降关系作为治疗的核心,该类方法不但受到多名中医大家的推崇,临床也多得效验。[结论]以“阴阳反作”作为部分内科疾病病机,以“升降失常”的视角审视临床内科疾病,可为临床治疗提供新思路、新方法。[Objective] To explore the pathogenesis of“abnormalities of ascending and descending”in traditional Chinese medical diseases and their corresponding treatment methods based on the theory of“Yin and Yang converse”. [Methods] By summarizing the concepts of Yin and Yang in“Yin and Yang converse”,further discussing the theory of“Yin and Yang converse”in terms of the three relationships between Yin and Yang,and analyzing the etiology and pathogenesis of“abnormalities of ascending and descending”and their corresponding treatments. [Results] According to the basic concepts of“Yin and Yang converse”and“abnormalities of ascending and descending”,these diseases can be classified into“Yin converse”“Yang converse”and“Yin and Yang converse”. These three categories can encompass a wide range of internal diseases,and all of them can be treated by regulating the relationship between ascending and descending as the core of treatment,and they have been clinically effective. [Conclusion] Taking“Yin and Yang converse”as the pathogenesis of some internal diseases and examining clinical internal diseases from the perspective of“abnormalities of ascending and descending”can provide new ideas and new methods for clinical treatment.
