摘 要:目的检测细胞外基质金属蛋白酶9(MMP-9)和肿瘤转移抑制基因(Kiss-1)在涎腺恶性多形性腺瘤中的表达,探讨其表达在恶性多形性腺瘤侵袭转移中的生物学意义。方法通过免疫组织化学技术SP法检测恶性多形性腺瘤不同大小的原发肿瘤之间;有、无区域性淋巴结转移的病例之间;有、无远处转移的病例之间MMP-9及Kiss-1表达的差异,采用χ^(2)检验及Spearman相关分析进行统计学分析。结果MMP-9在T1、T2、T3及有淋巴结转移、无淋巴结转移、有远处转移和无远处转移病例中的阳性表达率分别为88.9%,77.3%,81.3%,94.3%,41.7%,96.8%,50.0%。Kiss-1在T1、T2、T3及有淋巴结转移、无淋巴结转移、有远处转移和无远处转移病例中的阳性表达率分别为44.4%,45.5%,43.8%,31.4%,83.3%,25.8%,81.3%。MMP-9及Kiss-1在不同大小的原发肿瘤中的表达无差异,P>0.05。MMP-9在有淋巴结转移病例中的表达高于无淋巴结转移病例,二者差异具有显著统计学意义,P<0.01。MMP-9在有远处转移的恶性多形性腺瘤组织中的表达显著高于无远处转移的病例,二者差异具有显著统计学意义,P<0.01。Kiss-1在有淋巴结转移病例中的表达显著低于无淋巴结转移病例,二者差异具有显著统计学意义,P<0.01。Kiss-1在有远处转移的恶性多形性腺瘤组织中的表达显著低于无远处转移的病例,二者差异具有显著统计学意义,P<0.01。MMP-9在恶性多形性腺瘤组织中的表达与Kiss-1的表达具有负相关性,r=-0.324,P<0.05。结论MMP-9基因的高表达在涎腺恶性多形性腺瘤的区域淋巴结转移及远处转移中有着重要的促进作用。肿瘤转移抑制基因Kiss-1在恶性多形性腺瘤有区域淋巴结转移及有远处转移病例中的的低表达与涎腺恶性多形性腺瘤的侵袭转移生物学特性具有相关性。Objective To examine the expression of MMP-9 and Kiss-1 in malignant pleomorphic adenoma,of salivary gland and to investigate their biological significance in the invasiveness and metastasis.MethodsThe expression of MMP-9 and Kiss-1 was detected by immunohistochemistry Streptavidin-Peroxidase method in 47 cases of salivary malignant pleomorphic adenoma.The differences of MMP-9 and Kiss-1 expression in the differente size of tumor,with or without regional lymph nodes metastasis and with or without distant metastasis were compared byχ^(2)and Spearman test.ResultsThe positive rate of MMP-9 expression in T1,T2,T3,in cases with or without regional lymph nodes metastasis and in cases with or without distant metastasis was 88.9%,77.3%,81.3%,94.3%,41.7%,96.8%,50.0%.The positive rate of Kiss-1 expression in T1,T2,T3,in cases with or without regional lymph nodes metastasis and in cases with or without distant metastasis was 44.4%,45.5%,43.8%,31.4%,83.3%,25.8%,81.3%.There is no difference in the expression of MMP-9 and Kiss-1 in primary tumors of different sizes,P>0.05.A higher expression of MMP-9 was observed in cases with regional lymph nodes metastasis and distant metastasis.In cases with regional lymph nodes metastasis and distant metastasis the expression of Kiss-1 was remarkably decreased.Both difference has statistical significance,P<0.01.The expression of MMP-9 was negatively correlated with Kiss-1 expression in tissues of malignant pleomorphic adenoma,r=-0.324,P<0.05.Conclusion The higher expression of MMP-9 promote the invasiveness and metastasis of the salivary malignant pleomorphic adenoma.The lower expression of Kiss-1in cases with regional lymph nodes metastasis and distant metastasis is associated with biological characteristics of invasion and metastasis of salivary malignant pleomorphic adenoma.
