摘　　要：目的本研究拟通过检测口腔癌患者PD1免疫治疗前后外周血中T淋巴细胞各亚群的表达情况,探讨肿瘤患者机体免疫系统发生的变化。方法本项目拟采用2019年10月至2020年9月于河北医科大学第四医院60例经手术后病理证实口腔鳞癌的患者为研究对象,并给予PD1免疫治疗。用流式细胞仪检测CD4+T细胞和CD8+T细胞在T淋巴细胞中的百分比,计算CD4+T细胞/CD8+T细胞的比值。ELISA法检测口腔癌患者PD1免疫治疗前后血液中的IFN-γ和IL-2水平。结果经过PD1治疗后,口腔癌患者血液中IFN-γ、IL-2含量、CD4+T细胞百分比及CD4/CD8+T细胞比值都明显升高(P<0.001);在PD1治疗后,口腔癌患者血液中CD8+T细胞百分比的含量显著降低(P<0.001),口腔癌患者在接受PD1免疫治疗之后,患者的免疫功能显著增强。结论运用PD1免疫治疗口腔癌患者,治疗前后患者T细胞免疫功能明显增强,具有一定的临床应用价值与应用前景,为临床上应用针对口腔癌的免疫治疗提供理论依据。Objective In this study,the expression of T lymphocyte subsets in peripheral blood of patients with oral cancer before and after PD1 immunotherapy was detected to explore the changes in the immune system of patients with oral cancer.Methods In this study,60 patients with oral squamous cell carcinoma confirmed by operation and pathology in the Fourth Hospital of Hebei Medical University from October 2019 to September 2020 were selected as research objects and given PD1 immunotherapy.The percentage of CD4+T cells and CD8+T cells in T lymphocytes was measured by flow cytometry,and the ratio of CD4+T cells to CD8+T cells was calculated.Blood levels of IFN-γand IL-2 in patients with oral cancer were measured by ELISA before and after PD1immunotherapy.Results After PD1 treatment,blood levels of IFN-γ,IL-2,percentage of CD4+T cells and ratio of CD4/CD8+T cells were significantly increased(P<0.001).After PD1 treatment,the percentage of CD8+T cells in the blood of patients with oral cancer was significantly decreased(P<0.001),the immune function of patients with oral cancer was significantly enhanced after receiving PD1 immunotherapy.Conclusion The application of PD1 immunotherapy in the treatment of oral cancer patients did not decrease or even enhance the T cell immune status of patients after treatment,which has certain clinical application value and application prospect,and provides theoretical basis for the clinical application of immunotherapy for oral cancer.
