摘  要:目的观察种植体表面打磨抛光技术联合药物及Er:YAG激光治疗对种植体周围炎的短期临床疗效。方法选择2018年01月至2021年12月期间在我院口腔科收治的种植体周围炎患者共计113例作为研究对象,采用2×2析因设计的方式,随机数字表法将全部患者随机分为Ⅰ、Ⅱ、Ⅲ、Ⅳ组,Ⅰ组患者给予种植体打磨抛光疗法,Ⅱ组患者给予种植体打磨抛光与盐酸米诺环素联合疗法,Ⅲ组患者给予种植体打磨抛光与Er:YAG激光联合疗法,Ⅳ组患者给予种植体打磨抛光、盐酸米诺环素与Er:YAG激光联合疗法。于治疗前,治疗后第6周及治疗后第12周三个时间点对四组患者的种植体菌斑指数(PLI)、牙周探诊深度(PD)、龈沟出血指数(SBI)、龈沟液中炎症因子(TNF-α、IL-8、MMP-8)进行分析,以此作为临床疗效的评价依据。结果接受不同治疗方法的四组患者的PLI、PD、SBI均得到不同程度的改善,其中接受种植体打磨抛光、盐酸米诺环素与Er:YAG激光三种方式联合疗法的患者治疗效果明显优于单纯种植体打磨抛光疗法,也明显优于种植体打磨抛光与盐酸米诺环素或Er:YAG激光两种方式联合疗法的治疗效果。结论种植体打磨抛光技术与盐酸米诺环素及Er:YAG激光的联合疗法对于种植体周围炎的治疗具有一定的应用价值,值得推广和应用。Objective The study is aimed to analyze the short-term clinical efficacy of implant polishing combined with drug and laser therapy in the treatment of peri-implantitis.Methods A total of 113 peri-implantitis patients from Tangshan Union Hospital from January 2018 to December 2021 were enrolled in the study.A 2×2factorial design was used,and the patients were randomly divided into group I,groupⅡ,groupⅢand groupⅣ.Each group received different treatment methods.Patients in group I received implant polishingtherapy.Patients in groupⅡreceived implant polishing combined with minocyline hydrochloride therapy.Patients in groupⅢreceived implant polishing combined with Er:YAG laser therapy.And patients in groupⅣreceived implant polishing combined with minocyline hydrochloride and Er:YAG laser therapy.Implant plaque index(PLI),periodontal exploration depth(PD),sulcus bleeding index(SBI),and inflammatory factors in gingival crevicular fluid(TNF-α、IL-8、MMP-8)before and after treatment at week 6 and 12 in each group were analyzed as the basis for evaluation of clinical efficacy.Results The PLI,SBI and PD of the four groups were all improved to different degrees.However,the clinical efficacy of implant polishingcombined with minocyline hydrochloride and Er:YAG laser therapy was better than that of implant polishingtherapy,implant polishing combined with minocyline hydrochloridetherapy,implant polishing combined with Er:YAG laser therapy,and it had no adverse effects.ConclusionImplant polishing combined with minocyline hydrochloride and Er:YAG laser therapy is useful for peri-implantitis treatment and deserve application.
