摘　　要：目的 评估肝素结合蛋白(heparin binding protein,HBP)在急性胰腺炎合并感染的预测价值。方法 检索美国国立医学图书馆Pubmed数据库、荷兰医学文摘Embase数据库、Cochrane临床试验数据库、万方数据库、中国知网、维普数据库、中国生物医学数据库CBM数据库收录的自建库至今发表的有关肝素结合蛋白用于评估成人急性胰腺炎合并感染的中、英文文献。文献质量的评价采用诊断准确性研究的质量评价工具QUADAS-2,采用Meta-DIC和STATA 16.0统计学软件进行Meta分析,计算合并的敏感度、特异度、阳性似然比(PLR)、阴性似然比(NLR)和诊断优势比(DOR)等指标,绘制拟合受试者工作特征曲线(SROC)并计算曲线下面积(AUC);发表偏倚的检测采用Deek检验法。结果 13项研究中共1 732例急性胰腺炎患者纳入本次研究,纳入研究的文献质量相对适中。Meta分析结果显示:合并敏感度、合并特异度、合并PLR、合并NLR和合并DOR值及95%置信区间(95%CI)分别为0.62(95%CI:0.59~0.66)、0.84(95%CI:0.82~0.86)、4.96(95%CI:3.57~6.88)、0.35(95%CI:0.26~0.48)、17.12(95%CI:8.89~32.90);合并AUC为0.888,Cochran-Q=0.819。Deek检验显示纳入文献不存在发表偏倚。结论 HBP对急性胰腺炎合并感染有一定的预测价值。Objective To evaluate the predictive value of heparin binding protein in acute pancreatitis combined with infection.Methods The published Chinese and English literature relevant to heparin-binding proteins included in Pubmed database of USA National Library of Medicine,Netherlands Medical Abstracts Embase database,Cochrane Clinical Trials Database,Wanfang Database,CNK database,VIP database,and CBM database of Chinese Biomedical database was searched from the establishment of the databases to nowadays.The quality of literature was evaluated using QUADAS-2,a quality evaluation tool of diagnostic accuracy research.Meta-analysis was performed using Meta-DIC and STATA 16.0 statistical software to calculate the combined sensitivity,specificity,positive likelihood ratio(PLR),negative likelihood ratio(NLR),diagnostic odds ratio(DOR)and other indicators,plot the fitted receiver operating characteristic curve(SROC)and calculate the area under the curve(AUC).Publication bias was detected by Deek test.Results A total of 1732 cases with acute pancreatitis from 13 studies were included in this study,and the quality of the literature included in the studies was relatively moderate.The results of meta-analysis showed that the combined sensitivity,combined specificity,combined PLR,combined NLR and combined DOR values and 95%confidence intervals(95%CI)were 0.62(95%CI:0.59-0.66),0.84(95%CI:0.82-0.86),4.96(95%CI:3.57-6.88),0.35(95%CI:0.26-0.48),and 17.12(95%CI:8.89-32.90),respectively;the combined AUC was 0.888 and Cochran-Q=0.819.Deek test showed no publication bias in the included literature.Conclusion Heparin binding protein has certain predictive value for acute combined with infection.
