摘　　要：目的基于Murray定律研究布-加综合征(BCS)血管分叉形态结构,寻找可用于BCS早期风险预测的辅助性评价指标。方法收集96例BCS患者(BCS组)的术后MRI图像,32例正常志愿者(对照组)的MRI图像,三维重建下腔静脉及肝脏主干分支血管结构,测量下腔静脉与肝右、中、左血管直径及之间夹角,基于Murray定律提出分叉角度偏离量、截面积扩张率等评价指标,并应用受试者工作特征(ROC)曲线评价肝右静脉分叉角度偏离量对BCS的诊断价值。结果与对照组相比,BCS组下腔静脉截面积扩张率显著性升高,差异有统计学意义(P<0.05)。BCS组肝右静脉分叉角度偏离量升高,差异有统计学意义(P<0.05),经ROC曲线分析,cut-off值为0.521,曲线下面积(AUC)为0.771。结论肝右静脉分叉角度偏离量是血管结构的固有属性,可以定性地评价出肝右静脉血管偏离理想状态的情况,在BCS的诊断上具有一定的应用价值。Objective To study the structure morphology of vascular bifurcation in Budd-Chiari syndrome(BCS)based on Murray’s law to find auxiliary evaluation indicators for early risk prediction of BCS.Methods The MRI images of 96 patients with BCS(BCS group)and 32 normal volunteers(control group)were collected.The vascular structures of the inferior vena cava and its main branches in the liver were reconstructed in three dimensions.The diameters and angles between the inferior vena cava and the right,middle and left hepatic vein were measured.Based on Murray’s law,the bifurcation angle deviation and cross-sectional area expansion rate were proposed and regarded as the evaluation indictors.Receiver operating characteristic(ROC)curve was also used to evaluate the value of deviation of bifurcation angle of right hepatic vein in the diagnosis of BCS.Results Compared with the control group,the dilatation rate of inferior vena cava cross-sectional area in BCS group was significantly increased(P<0.05).The high angle deviation of the right hepatic vein in the BCS group was significant(P<0.05).ROC curce showed the deviation of the right hepatic vein bifurcation angle with a cut-off value of 0.521,and with the area under the curve(AUC)of 0.771.Conclusion The deviation of the right hepatic vein bifurcation angle is an inherent attribute of the vascular structure,which can be used to qualitatively evaluate the deviation of the right hepatic vein from the ideal state,and has certain application value in the diagnosis of BCS.
