自锁托槽矫正器与传统托槽矫正器在牙周病患者正畸治疗中的应用效果比较

摘　　要：目的探讨牙周病正畸治疗患者采取自锁托槽与传统托槽矫正器治疗的效果。方法抽取2021年1月至2022年6月河南省直第三人民医院收治的牙周病正畸治疗患者78例。根据矫正器类型分为对照组(传统托槽)和观察组(自锁托槽),每组39例。比较两组治疗效果,比较两组治疗前后牙周指数[菌斑指数(PLI)、牙龈沟出血指数(SBI)、牙龈指数(GI)]和龈沟液指标[龈沟液量、白细胞介素-1β(IL-1β)及肿瘤坏死因子-α(TNF-α)],比较两组不良反应发生率。结果观察组总有效率(97.44%,38/39)高于对照组(79.49%,31/39),P<0.05。治疗前,两组牙周指数、龈沟液指标比较差异未见统计学意义(P>0.05);治疗后,观察组PLI、GI、SBI、龈沟液量、IL-1β、TNF-α均低于对照组(P均<0.05)。观察组并发症发生率(5.12%,2/39)与对照组(12.82%,5/39)比较差异未见统计学意义(P>0.05)。结论与传统托槽矫正器比较,自锁托槽矫正器用于牙周病正畸治疗中,可更好地改善牙周指数与龈沟液指标,提高临床疗效,且不会显著增加不良反应。Objective To investigate the effect of self-ligating brackets and traditional brackets in orthodontic treatment for periodontal disease.Methods Seventy-eight patients with periodontal disease who underwent orthodontic treatment in the Third People’s Hospital of Henan Province from January 2021 to June 2022 were selected.And they were divided into control group(treated by traditional brackets)and observation group(treated by self-ligating brackets)according to the type of orthodontic appliance,with 39 cases in each group.The treatment effects of two groups were compared.The periodontal indexes,including plaque index(PLI),gingival sulcus bleeding index(SBI)and gingival index(GI),and gingival crevicular fluid indexes,including gingival crevicular fluid volume,interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)were compared between the two groups before and after treatment.The incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group(97.44%,38/39)was higher than that of the control group(79.49%,31/39),P<0.05.Before treatment,there was no statistically significant difference in periodontal indexes and gingival crevicular fluid indexes between the two groups(P>0.05).After treatment,PLI,GI,SBI,gingival crevicular fluid volume and levels of IL-1βand TNF-αof the observation group were lower than those of the control group(all P<0.05).There was no significant difference in the incidence of complications between the observation group(5.12%,2/39)and the control group(12.82%,5/39),P>0.05.Conclusions Compared with traditional orthodontic brackets,self-ligating brackets,in the orthodontic treatment for periodontal disease,can better improve periodontal indexes,gingival crevicular fluid indexes and clinical efficacy,without significant increase in adverse reactions.
