晚期肺癌中医证型与其免疫功能状态及预后的相关性分析

摘　　要：目的探讨晚期肺癌中医证型与免疫指标的相关性,以及两者对晚期肺癌患者预后的影响。方法对患者进行辨证分型,检测不同证型患者治疗前、治疗后三个月外周血T细胞亚群数目,应用COX回归分析影响晚期肺癌预后的独立影响因素。结果共收集晚期肺癌患者73例,其中不同CD4的晚期肺癌患者中医证型分布比较,具有统计学差异(P<0.05),不同治疗方式、KPS评分、CD3、CD4和CD8的晚期肺癌患者的无进展生存期比较,具有统计学差异(P<0.05),中医证型、治疗方式是影响晚期肺癌患者预后的独立危险因素(P<0.05)。结论根据辨证分型采取合适的治疗方式,可延长晚期肺癌患者的生存期。Objective To explore the correlation between TCM syndrome types and immune indexes in patients with advanced lung cancer,and the effect of both on prognosis of patients with advanced lung cancer.Methods The patients were classified according to syndrome differentiation.The number of T cell subsets in peripheral blood of patients of different syndrome types was detected before and after treatment for three months.The correlation between TCM syndrome types and other objective indexes was discussed.The independent influencing factors of prognosis of advanced lung cancer were analyzed by COX regression.Results A total of 73 patients with advanced lung cancer were collected,in which the distribution of TCM syndrome types of different CD4 patients was statistically different(P<0.05),and the progression-free survival time of patients with different treatment methods,KPS score,CD3,CD4 and CD8 was statistically different(P<0.05).TCM syndrome types and treatment methods were independent risk factors affecting the prognosis of patients with advanced lung cancer(P<0.05).Conclusion Appropriate treatment should be adopted according to syndrome differentiation to prolong the survival time of patients with advanced lung cancer.
