膀胱癌组织中DNMT3b、TILs表达及与其预后的关系

摘　　要：目的分析膀胱癌组织中DNA甲基转移酶3b(DNMT3b)、肿瘤浸润淋巴细胞(TILs)表达及与其预后的关系。方法选取79例膀胱癌患者,对其进行回顾性分析。79例患者均测定DNMT3b、TILs表达水平,并随访2年,根据患者2年内肿瘤复发情况将其分为未复发组(n=48)及复发组(n=31),比较未复发组与复发组DNMT3b、TILs表达水平,使用COX分析模型分析DNMT3b、TILs表达水平与肿瘤复发的相关性。使用受试者工作曲线(ROC)分析DNMT3b、TILs表达水平联合预测膀胱癌患者肿瘤复发的价值。分别比较不同DNMT3b表达水平、不同TILs表达水平患者无瘤生存期(DFS)。结果未复发组患者DNMT3b表达水平显著低于复发组患者,TILs表达水平显著高于复发组患者(P<0.05)。经COX模型分析,DNMT3b表达水平与肿瘤复发呈正相关,TILs表达水平与肿瘤复发呈负相关(P<0.05)。ROC曲线中,DNMT3b、TILs表达水平联合预测膀胱癌患者肿瘤复发的曲线下面积(AUC)为0.855,敏感度为85.42%,特异度为77.42%。DNMT3b高表达患者DFS显著短于DNMT3b低表达者(P<0.05);TILs高表达患者DFS显著长于TILs低表达者(P<0.05)。结论膀胱癌组织中DNMT3b、TILs表达水平与患者预后密切相关,可为术后早期识别高危患者并规划诊疗方案提供证据支持。Objective To analyze the expressions of DNA methyltransferase 3b(DNMT3b)and tumor-infiltrating lymphocytes(TILs)in bladder cancer tissues and their relationship with prognosis.Methods 79 patients with bladder cancer were selected for retrospective analysis.The expression levels of DNMT3b and TILs were detected among 79 patients,and the patients were followed up for 2 years.According to the tumor recurrence within 2 years,the patients were divided into non-recurrence group(n=48)and recurrence group(n=31).The expression levels of DNMT3b and TILs were compared between non-recurrence group and recurrence group.COX analysis model was used to analyze the correlation of expression levels of DNMT3b and TILs with tumor recurrence.Receiver operating characteristic curve(ROC)was adopted to analyze the predictive value of combined expression levels of DNMT3b and TILs on predicting tumor recurrence in patients with bladder cancer.Disease-free survival(DFS)was compared among patients with different expression levels of DNMT3b and TILs.Results The expression level of DNMT3b in non-recurrence group was significantly lower than that in recurrence group while the expression level of TILs was significantly higher than that in recurrence group(P<0.05).According to COX analysis model,the expression level of DNMT3b was positively correlated with the tumor recurrence,but the expression level of TILs was negatively correlated with the tumor recurrence(P<0.05).In ROC curves,the area under curve(AUC),sensitivity and specificity of combined expression levels of DNMT3b and TILs in predicting tumor recurrence in patients with bladder cancer were 0.855,85.42%and 77.42%.The DFS in patients with high DNMT3b expression was significantly shorter than that in patients with low DNMT3b expression(P<0.05).DFS in patients with high TILs expression was significantly longer than that in patients with low TILs expression(P<0.05).Conclusion The expression levels of DNMT3b and TILs in bladder cancer tissues are closely related to the prognosis of patients,
