乳腺癌患者不选择保留乳房手术的相关影响因素分析

摘　　要：目的分析乳腺癌患者不选择保留乳房手术的影响因素。方法回顾性分析收治的98例乳腺癌患者的临床资料,根据其手术方式的不同将其分为保留乳房手术组与乳腺癌根治术组;运用回顾性调查分析方式,收集整理患者的年龄、体重指数(BMI)等一般资料,以此分析影响患者不选择保留乳房手术治疗的相关因素。结果单因素分析结果显示:年龄、体重指数(BMI)、婚姻状况、生育史、临床分期、医疗保险与乳腺癌患者不选择保留乳房手术治疗无关(P>0.05);家庭人均可支配收入、居住地、癌症家族史、乳腺良性疾病史、外科医生建议与患者不选择保留乳房手术治疗密切相关(P<0.05)。多因素Logistic回归分析结果显示:家庭人均可支配收入<2.5万、居住在农村、有癌症家族史、有乳腺良性疾病史、外科医生不建议保乳为乳腺癌患者不选择保留乳房手术治疗的独立影响因素(P<0.05)。结论家庭人均可支配收入≤2.5万、居住在农村、外科医生不建议保乳等为乳腺癌患者不选择保留乳房手术治疗的主要影响因素,临床需予以高度重视。Objective To analyze the influencing factors of breast cancer patients not choosing breast conserving surgery.Methods The clinical data of 98 patients with breast cancer were retrospectively analyzed,and they were divided into breast conserving surgery group and breast cancer radical surgery group according to the different surgical methods;Using a retrospective survey analysis method,collect and organize general data such as age and body mass index(BMI)of patients,in order to analyze the relevant factors that affect patients'unwillingness to choose breast conserving surgery treatment.Results Univariate analysis showed that age,body mass index(BMI),marital status,childbearing history,clinical stage,and medical insurance were not related to breast cancer patients who did not choose breast conserving surgery(P>0.05);The per capita disposable income of the family,place of residence,family history of cancer,history of benign breast diseases,and advice from surgeons were closely related to patients not choosing breast conserving surgery(P<0.05).The results of multivariate logistic regression analysis showed that:the family's per capita disposable income was less than 25000,living in rural areas,had family history of cancer and benign breast disease,and surgeons did not recommend breast conservation were independent influencing factors for breast cancer patients not to choose breast conservation surgery(P<0.05).Conclusion The main influencing factors for breast cancer patients not to choose breast conserving surgery are family disposable income≤25000,living in rural areas,and surgeons not recommending breast conserving.
