皮肤基底细胞癌与基底细胞乳头状瘤的超声特征及鉴别特征

摘　　要：目的探讨皮肤基底细胞癌(BCC)与基底细胞乳头状瘤(BCP)的超声特征及鉴别诊断特征。方法回顾性分析皮肤BCC和BCP的超声表现,提取声像图特征并绘制受试者工作特征(ROC)曲线计算曲线下面积(AUC)评价鉴别诊断效能。结果皮肤BCC和BCP在病灶累及层次、最大径、边界、形态、点状强回声、角质层增厚均匀度、基底方面有一定特征,差异比较具有统计学意义(P<0.05)。两者鉴别上累及层次诊断效能最佳,其AUC为0.861,灵敏度为77.2%,特异度为100%,其次是点状强回声(AUC为0.758,灵敏度为77.8%,特异度为73.9%)。结论皮肤BCC和BCP具有一定的超声声像图特征,其中病灶累及层次、内部点状强回声在两者的鉴别诊断中有较高价值。Objective To investigate the ultrasonographic features and differential diagnosis between Basal-cell carcinoma(BCC)and basal cell papilloma(BCP).Methods The ultrasonic manifestations of skin BCC and BCP were analyzed retrospectively.The ultrasonic images were extracted and the receiver operating characteristic(ROC)curve was drawn to calculate the area under the curve(AUC)to evaluate the differential diagnostic efficacy.Results BCC and BCP of the skin had certain characteristics in the lesion involved layers,maximum diameter,boundary,shape,internal punctate strong echo,thickness uniformity of stratum corneum,the base,and the differences were statistically significant(P<0.05).The best diagnostic performance was the lesion involved lagen,the AUC was 0.861,sensitivity was 77.2%,specificity was 100%,followed by internal punctate strong echo(AUC 0.758,sensitivity 77.8%,specificity 73.9%).Conclusions BCC and BCP of the skin have certain ultrasonographic characteristics,in which the lesion involved layers,internal punctate strong echo have high value in the differential diagnosis of the two.
