摘 要:目的 探讨宫颈癌前病变患者组织中P53、CD44v6的表达异常与HPV感染的交互作用。方法 选取2021年1月至2023年1月期间于青岛市第八人民医院妇科接受手术切除的癌前病变组织石蜡标本70例作为病例组,其中宫颈低级别上皮内病变(ISIL)标本和宫颈高级别上皮内病变(HSIL)标本各为35例。另纳入同期因子宫肌瘤性子宫切除术的正常宫颈组石蜡标本作为对照(正常宫颈组)。检测各组患者组织中的P53、CD44v6蛋白表达水平,并统计HPV感染率。结果 HSIL组中的CD44v6阳性表达率较ISIL组、正常宫颈组均明显偏高;HSIL组中的P53蛋白阳性表达率高于ISIL组和正常宫颈组(P<0.05)。HPV与P53蛋白、CD44v6均存在病毒-肿瘤蛋白表达的交互作用,可明显增加LSIL向HSIL发展的风险(P<0.05)。P53-HPV、HPV-CD44v6的病毒-肿瘤蛋白表达的交互作用是影响HSIL向宫颈癌恶化的危险因素(P<0.05)。结论 宫颈癌前病变患者发生HPV感染与其组织中P53、CD44v6表达异常存在一定的相关性,可在一定程度上增加疾病向恶性发展的风险。Objective Exploring the interaction between abnormal expression of P53 and CD44v6 in cervical precancerous lesions and HPV infection.Methods From January 2021 to January 2023,70 paraffin samples of precancerous lesions were selected as the case group,including 35 samples of low-grade cervical intraepithelial lesion(ISIL)and 35 samples of high-grade cervical intraepithelial lesion(HSIL).In addition,paraffin samples of normal cervical group due to hysteromyoma hysterectomy in the same period were included as control(normal cervical group).The expression levels of P53 and CD44v6 protein in tissues of patients in each group were detected,and the infection rate of HPV was counted.Results The positive expression rate of CD44v6 in HSIL group was significantly higher than that in ISIL group and normal cervical group.The positive expression rate of P53 protein in HSIL group was higher than that in ISIL group and normal cervical group(P<0.05).HPV interacts with P53 protein and CD44v6 protein expression,which can obviously increase the risk of LSIL developing into HSIL(P<0.05).The interaction of P53-HPV and HPV-CD44v6 virus-tumor protein expression is a risk factor for the deterioration of HSIL to cervical cancer(P<0.05).Conclusion HPV infection in patients with cervical precancerous lesions is related to the abnormal expression of P53 and CD44v6 in tissues,which can increase the risk of malignant development of the disease to some extent.
