吲哚菁绿辅助PHACO+IOL植入术治疗白内障的效果及对眼压、角膜内皮细胞的影响

摘　　要：目的探讨吲哚菁绿辅助白内障超声乳化吸除术(PHACO)+人工晶状体(IOL)植入术治疗白内障的效果及对眼压、角膜内皮细胞的影响。方法选择2020年1月到2022年10月接受PHACO+IOL植入术治疗的白内障患者86例,按照随机数表法分为观察组与对照组,各43例。两组均接受PHACO+IOL植入术治疗,观察组术中使用吲哚菁绿,对照组不使用吲哚菁绿。比较两组术中撕囊成功率,及术前、术后1周及1个月最佳矫正视力(BCVA)、眼压及角膜内皮细胞数量及术后并发症发生率。结果观察组术中撕囊成功率为93.02%,明显高于对照组76.74%,差异有统计学意义(P<0.05);两组术前、术后1周及1个月时BCVA和眼压比较,差异均无统计学意义(P>0.05);观察组术后1周、术后1个月时角膜内皮细胞数量分别为(2,194.34±173.09)个/mm^(2)、(2,214.39±155.81)个/mm^(2),均明显多于对照组(1,991.84±157.03)个/mm^(2)、(2,021.23±163.42)个/mm^(2),差异均有统计学意义(P<0.05);观察组术眼对手术的反应率为23.26%,明显低于对照组46.51%,差异有统计学意义(P<0.05)。结论吲哚菁绿辅助用于白内障患者PHACO+IOL植入术中可提高撕囊成功率,并能减少角膜内皮损伤,且不影响眼压,安全可靠,值得临床推广。Objective To investigate the effect of indocyanine green assisted phacoemulsification(PHACO)and intraocular lens(IOL)implantation in the treatment of cataract and its effects on intraocular pressure and corneal endothelial cells.Methods Selected 86 cataract patients who received PHACO+IOL implantation in our hospital from January 2020 to October 2022,and divided them into the observation group and the control group by the random number table,43 patients in each group.Both groups were treated with PHACO+IOL implantation,and indocyanine green was used in the observation group,while indocyanine green was not used in the control group.The success rate of capsulorhexis during operation was compared between the two groups,and the best corrected visual acuity(BCVA),intraocular pressure and corneal endothelial cell number before operation,after operation 1 week and 1 months and postoperative complication rate were compared.Results The successful rate of capsulorhexis in the observation group was 93.02%,which was significantly higher than the control group 76.74%,with statistically significant(P<0.05);there was no significant difference in BCVA and intraocular pressure between the two groups at before operation,after operation 1 week and 1 months(P>0.05);at after operation 1 week and1 month,the number of corneal endothelial cells in the observation group were(2,194.34±173.09)number/mm^(2)and(2,214.39±155.81)number/mm^(2),which were significantly higher than those in the control group(1,991.84±157.03)number/mm^(2)and(2,021.23±163.42)number/mm^(2),and the difference were statistically significant(P<0.05);the reaction rate of operative eyes in the observation group was 23.26%,which was significantly lower than that in the control group 46.51%with statistically significant(P<0.05).Conclusion Indocyanine green assisted in PHACO+IOL implantation in cataract patients can improve the success rate of capsulorhexis,reduce corneal endothelial damage,and do not affect intraocular pressure,it's safe and reliable,and is worthy of c
