320排螺旋CT在十二指肠憩室诊断中的应用及临床意义

摘　　要：目的评价320排螺旋CT用于诊断十二指肠憩室的临床意义。方法分析2021年5月至2022年5月北京市平谷区医院经上消化道造影或内镜检查证实的并已行320排螺旋CT检查的71例十二指肠憩室患者的临床资料,分析十二指肠憩室的直接征象、典型表现及伴发征象。结果71例患者共检出十二指肠憩室82个,单发61例,双发9例,多发1例。发生在十二指肠降段56例,水平段5例,降段与水平段均有10例。憩室直径<1.0 cm 3例,1.0~2.0 cm 29例,>2.0 cm 39例。憩室内成分为纯气体19个(23.17%);气液混合、气-液平面20个(24.39%);气体、食物残留混杂成分43个(52.44%)。71例患者中有31例合并胆道胰腺相关疾病。合并胆胰相关疾病组憩室位于降段的比例高于未合并胆胰相关疾病组,差异有统计学意义(P<0.05);两组的憩室大小占比比较,差异无统计学意义(P>0.05)。结论320排螺旋CT可很好地诊断十二指肠憩室,能及时发现相关的胆胰类并发症,具有重要的临床意义。Objective To evaluate the clinical significance of 320 spiral CT on diagnosis of duodenal diverticulum.Methods The clinical data of 71 patients with duodenal diverticulum who underwent 320 spiral CT confirmed by upper gastrointestinal tract imaging or endoscopy from May 2021 to May 2022 in Pinggu District Hospital,Beijing were analyzed,and the direct signs,typical manifestations and associated signs of duodenal diverticulum were analyzed.Results There were a total of 82 diverticula in 71 patients with duodenal diverticulum,61 of whom were single diverticulum,9 of whom were double diverticula,and 1 of whom were multiple diverticula.56 were in the decent section of the duodenum,5 were in the horizontal section of the duodenum,10 were in two segments.3 were<1.0 cm in diameter,29 were 1.0-2.0 cm,and 39 were>2.0 cm.19(23.17%)diverticula contained gas alone.20(24.39%)diverticula contained gas-fluid level,43(52.44%)diverticula contained gas and food residues.31 complicated with bile duct and pancreatic diseases in the 71 patients.The proportion of decent section of duodenum in group of complicated with bile duct and pancreatic diseases higher than without bile duct and pancreatic diseases,and the difference was statistically significant(P<0.05).There was no significant difference in the proportion of diverticulum size between the two groups(P>0.05).Conclusion It is useful to diagnosis duodenal diverticulum by 320 spiral CT.It can detect the complication related to biliopancreatic timely and have a high value clinical significance.
