Effects of Different Frequency of MECT Combined with Second-generation Antipsychotics in the Treatment of Inpatients with Schizophrenia

摘  要:目的:研究不同频次改良电抽搐治疗(MECT)联合第二代抗精神药物治疗精神分裂症住院患者的效果。方法:选取上饶市第三人民医院老年科2019年1月—2022年1月收治的精神分裂症住院患者80例,按照随机数字表法将其分为药物组(26例)、高频组(27例)、低频组(27例)。药物组予以单纯药物治疗,高频组在此基础上予以3次/周的MECT治疗,低频组在药物组基础上予以1次/周的MECT治疗。观察三组临床疗效、认知功能及神经递质水平。结果:治疗后,低频组、高频组的阳性和阴性症状量表(PANSS)评分均低于药物组(P<0.05);低频组的PANSS评分低于高频组(P<0.05)。低频组、高频组的临床总有效率均高于药物组(P<0.05);低频组、高频组的临床总有效率比较,差异无统计学意义(P>0.05)。治疗后,低频组、高频组各维度的MATRICS认知成套测验(MCCB)评分均高于药物组(P<0.05);低频组的各维度MCCB评分均高于高频组(P<0.05)。治疗后,低频组、高频组的5-羟色胺(5-HT)、多巴胺(DA)、去甲肾上腺素(NE)水平均高于药物组(P<0.05);低频组的5-HT、DA、NE水平均高于高频组(P<0.05)。结论:与高频次MECT联合第二代抗精神药物治疗精神分裂症住院患者相比,低频次MECT联合第二代抗精神药物治疗可显著改善患者认知功能,有效调节神经递质水平。但两者的临床总有效率无差异,可能受限于试验样本量的因素。Objective:To study the effect of different frequency of modified electroconvulsive therapy(MECT)combined with second-generation antipsychotics in the treatment of inpatients with schizophrenia.Method:A total of 80 inpatients with schizophrenia admitted to the Geriatrics Department of Shangrao Third People's Hospital from January 2019 to January 2022 were selected,and were divided into drug group(26 cases),high-frequency group(27 cases)and low-frequency group(27 cases)according to random number table method.The drug group was given simple drug treatment,and the high-frequency group was given MECT treatment 3 times/week on the basis of this,the low-frequency group was given MECT once a week on this basis of drug group.The clinical efficacy,cognitive function and neurotransmitter level of the three groups were observed.Result:After treatment,the positive and negative syndrome scale(PANSS)scores of low-frequency group and high-frequency group were lower than that of drug group(P<0.05);the PANSS score of low-frequency group was lower than that of high-frequency group(P<0.05).The total clinical effective rates of the low-frequency group and the high-frequency group were higher than that of the drug group(P<0.05);there was no difference between the low-frequency group and the high-frequency group(P>0.05).The all dimensions MATRICS consensus cognitive battery(MCCB)scores of low-frequency group and high-frequency group were higher than those of drug group(P<0.05),and the all dimensions MCCB scores of low-frequency group were higher than those of high-frequency group(P<0.05).After treatment,the levels of 5-hydroxytryptamine(5-HT),dopamine(DA)and norepinephrine(NE)in low-frequency group and high-frequency group were higher than those in drug group(P<0.05),and the levels of 5-HT,DA and NE in low-frequency group were higher than those in high-frequency group(P<0.05).Conclusion:Compared with high-frequency MECT combined with second-generation antipsychotics in the treatment of schizophrenia inpatients,low-frequency MECT combin
