摘 要:目的 评价以CT为主的各种影像学检查方法对肾上腺肿瘤的临床诊断价值 ,为临床医师合理有序地运用各种影像学检查方法提供参考。材料与方法 以手术病理为依据 ,用比较影像学的方法 ,对各种影像学检查方法的定位、定性诊断正确率对比分析。结果 94例肾上腺肿瘤中 ,良性 86例 (内含囊性占位 10例 ) ,恶性 8例 ,IVP、BUS、CT、CDFI、MRI的术前定位诊断正确率分别为 4 2 5 %、91 5 %、96 7%、94 1%、92 9% ,定性诊断正确率分别为 15 %、82 9% (鉴别囊、实性 )、74 7%、6 4 7%、78 6 % ,综合影像组则高达 10 0 %及 85 1%。结论 IVP定位、定性诊断率低 ;BUS对囊性、实性的识别力高 ;IVP +BUS可作为肾上腺肿瘤筛选手段 ;CT、CDFI、MRI对肾上腺肿瘤的定位诊断正确率高 ,定性诊断正确率相近 ,对鉴别良、恶性有很高的诊断价值 ,综合影像检查可发挥多种检查的互补作用 ,明显提高肾上腺肿瘤的诊断正确率。Objective To evaluate the value of medical imaging in the diagnosis of adrenal neoplasms. Methods To evaluate the localizing and qualitative diagnostic accuracy of imaging examination, the results of pathologic diagnosis and image analysis were compared. Results 94 patients with adrenal neoplasms (86 benign, 8 malignant) were included in the study. The preoperative localizing diagnostic accuracy of IVP, BUS, CT, CDFI, MRI and co-image analysis were 42.5%, 91.5%, 96.7%, 94.1%, 92.9% and 100% respectively. The preoperative qualitative diagnostic accuracy of IVP, BUS, CT, CDFI, MRI and co-image examination were 15%, 82.9%, 74.7%, 64.7%, 78.6% and 85.1% respectively. Conclusion The diagnostic accuracy of IVP was poor. BUS was valuable in differentiating cystic with solid adrenal neoplasms. IVUS combined with BUS can be used as screening test for adrenal neoplasms. CT, CDFI and MRI with high diagnostic accuracy were valuable in differentiating benign with malignant adrenal neoplasms. Integration of imaging examination can increase the diagnostic accuracy in adrenal neoplasms diagnosis.
