某院2018—2021年麻醉药品的使用情况及合理性分析

摘　　要：目的调查某医院2018—2021年门急诊麻醉药品使用情况并评价其合理性,为麻醉药品的临床合理应用及科学管理提供参考。方法利用医院信息系统调取某院2018—2021年患者麻醉药品的使用数据,对不同药物的销售总金额、构成比、药品的消耗量、用药频度、限定日费用及药品排序比(B/A)进行统计处理和分析,同时依照相关麻醉药品及处方管理办法对某院2019—2021年门急诊麻醉药品处方进行合理性评价。结果(1)用药频度排前3位的分别是枸橼酸舒芬太尼、硫酸吗啡缓释片和盐酸羟考酮缓释片10 mg;各麻醉药品的药品日均费用值基本稳定,排名前3位的分别是注射用盐酸瑞芬太尼、盐酸羟考酮缓释片10 mg、盐酸羟考酮缓释片40 mg;2018—2021年盐酸吗啡片、盐酸羟考酮缓释片排序比(B/A)值接近于1.00,同步性较好,其他药物的B/A值在0.25~3之间。(2)2019年5月—2020年4月、2020年5月—2021年4月门急诊麻醉药品处方不合理占比分别为2.3%、0.55%,不合理占比降低,处方合格率提高。结论某院2018—2021年麻醉药品使用情况基本符合规范,管理质量有所提升,但是仍然存在一定问题,今后仍需继续严格监管。Objective To investigate the application status of narcotic drugs in outpatient and emergency department of a hospital from 2018 to 2021 and evaluate the application rationality,and to provide reference for rational clinical use and scientific management of narcotic drugs.Methods The data of drug use in a hospital from 2018 to 2021 were collected through the hospital information system.The total sales amount,component ratio,drug consumption,frequency of drug use(DDDs),defined daily cost(DDC),and drug sequence ratio(B/A)of narcotic drugs were analyzed.The rationality of outpatient and emergency narcotic drugs prescriptions of a hospital from 2019 to 2021 was evaluated according to the relevant narcotic drugs and prescription management regulations.Results(1)The top 3 narcotic drugs ranked by DDDs were sufentanil citrate,morphine sulfate sustained release tablets and oxycodone hydrochloride sustained release tablets 10 mg.DDC of narcotic drugs were basically stable,and the top 3 were remifentanil hydrochloride for injection,oxycodone hydrochloride sustained-release tablets 10 mg,and oxycodone hydrochloride sustained-release tablets 40mg.B/A values of morphine hydrochloride tablets and oxycodone hydrochloride sustained-release Tablets during 2018-2021 were close to 1.00,indicating good synchronization,and the B/A values of other drugs were 0.25-3.(2)The proportion of rrational prescriptions for outpatient and emergency narcotic drugs were 2.3% from May 2019 to April 2020 and 0.55% from May 2020 to April 2021 respectively.The proportion of irrational prescriptions decreased and the qualified rate of prescriptions was increased.Conclusion The use of narcotic drugs in a hospital from 2018 to 2021 basically met to the norms,and the management quality has been improved.But there are still some problems that need strict supervision is still needed in the future.
