摘 要:目的探究儿科中青年教师的临床带教现状并分析存在问题。方法采用问卷调查方法,选取2022年6月中山大学孙逸仙纪念医院儿科在岗主治及以下医师为研究对象,收集并统计分析其带教资质、教学活动实施情况、实习评价及建议等资料。结果共65人参与问卷调查,参与度为82.2%。带教老师主要来自住培医师及研究生队伍(49.2%),41.5%的带教老师具有3年及3年以上带教经历,并参加过系统的师资培训,多数带教老师带教意识一般(53.8%)、对实习大纲的熟悉程度基本了解(56.9%)。日常教学活动主要包括:病史采集与病历书写(78.5%)、教学查房(67.7%)和技能操作(76.9%)。同时还开展医患沟通技巧(84.6%)、人文关怀与爱伤意识(78.5%)、科普与健康宣教(44.6%)、医疗质量安全核心制度(41.5%)、科研思维(7.7%)等拓展教学。对于管床病例,对实习生的要求主要是了解(63.1%),对于特殊或典型病例,56.9%会积极主动施教,而当病种或技能操作不能满足实习大纲要求时,仅有4.6%带教老师会选择忽略不管,其他带教老师则会通过讲授理论知识或情景演练(73.8%)、引领实习生去其他病区学习(56.9%)、历史案例查询与学习(49.2%)、借助多媒体教材(33.8%)的方式进行补救。对实习现状的评价为非常满意、满意、一般、非常不满意分别占3.1%、29.2%、64.6%、3.1%。缺乏积极主动性(55.4%)是实习生存在的最主要问题,而研究生入学考试(44.6%)、专业认同感(29.2%)则是其中主要影响因素。为加强实习管理工作,建议从科室、带教老师多个层面着手。结论中青年教师是儿科临床带教的主力,在教学活动中具有一定自主性,但缺乏统一标准,如何加强儿科临床教学规范化管理、调动实习生临床实践的积极性是目前面临的难题。Objective To explore the current clinical teaching situation of young and middle-aged pediatric teachers and analyzing existing problems.Methods A questionnaire survey was conducted to collect information on the teaching qualifications,implementation of teaching activities,internship evaluation and recommendations of physicians in the department of pediatrics at Sun Yat-sen Memorial Hospital in June 2022.Results A total of 65 teaching staff participated in the questionnaire,with a participation rate of 82.2%.The teaching staff were mainly from the team of standardized training physicians and postgraduates(49.2%),41.5%had 3 years or more experience in teaching and had attended systematic relevant training.And most teachers had a general awareness of teaching(53.8%)and a basic understanding of the internship manual(56.9%).The main daily teaching activities included history taking and medical record writing(78.5%),teaching rounds(67.7%)and clinical skills practice(76.9%).Extended teaching of communication skills(84.6%),humanistic care and injury awareness(78.5%),science and health promotion(44.6%),core medical quality and safety system(41.5%)and research thinking(7.7%)were also conducted.The interns were mostly asked to have a general understanding of common cases(63.1%),and 56.9%of them received special or typical case studies.Whereas only 4.6%teachers would choose to ignore when a disease or skill operation failed to meet the requirements of the internship syllabus,others would remedy the situation by teaching theoretical knowledge or scenariobased exercises(73.8%),leading interns to other wards(56.9%),historical case searching and learning(49.2%),and using multimedia teaching materials(33.8%).The current status of the internship was rated as very satisfactory,satisfactory,fair and very unsatisfactory by 3.1%,29.2%,64.6%and 3.1%respectively.Lack of motivation(55.4%)was the most important problem for interns,and postgraduate examinations(44.6%)and professional identity(29.2%)were the main influencing factors.It was
