摘　　要：临床思维训练是专业型硕士生培养中非常重要的一个环节,但是因为学生病史采集和体格检查基础薄弱、主动学习能力和思考能力弱、导师对临床思维训练不重视等原因,难以达到理想效果。导师在专业型硕士的培养中起至关重要的作用,定期进行临床思维训练是住院医师规范化培训有力的补充。小规模限制性在线课程(small private online course teaching,SPOC)教学模式是一种新型教学模式,导师利用在线平台进行专业知识的教学,结合线下组会,定期组织分段式病例讨论,有助于巩固学生专业知识,引导学生利用病史、患者症状、体格检查、实验室检查进行分析推理,做出诊断和治疗决策,较好地锻炼了临床思维能力。基于SPOC教学模式,临床思维训练可提高学生病史采集和体格检查水平、提高主动学习和思考能力、充分发挥导师的引导作用,具有良好的教学效果。Clinical thinking training is an important part in professional master education.However,it is difficult to achieve ideal effect due to insufficient competence of medical history taking and physical examination,inadequate capacity of active learning and independent thinking of postgraduate students and lack of attention to clinical thinking training paid by tutors.Actually,tutors play a vital role in the education of postgraduate students.It is necessary to perform regular clinical thinking training for students.Small private online course teaching(SPOC)is a novel teaching mode,by which tutors are able to use the online platform to teach professional knowledge combining with the segmented case discussion in the offline group meetings.The SPOC teaching helps to consolidate knowledge and guide the students to use the medical history,physical examination and laboratory findings for analysis and reasoning,thus make diagnosis and treatment decision.Based on the SPOC teaching mode,the tutor-guided clinical thinking training is helpful to improve postgraduate students’ability of medical history taking and physical examination,cultivate their thinking ability of active learning and independent thinking and take full advantage of tutors,showing satisfactory results in teaching practice.
