摘  要:目的:应用椎体束CT探讨在上颌双根第一前磨牙颊根腭侧凹陷的发生率、腭侧凹陷厚度的对称性,以及与性别、年龄及不同牙根部位的关系,为临床诊疗提供参考。方法:选取本院口腔科259例患者的锥形束CT(CBCT)扫描数据,按年龄分成4组,测量颊根管腭侧壁在不同牙根部位各壁厚度并分析结果。结果:腭侧凹陷的发生率为77.9%;左右两侧颊根腭侧壁厚度相似,无显著差异(P>0.05);随年龄增长不同牙根部位的腭侧壁厚度均增加且有显著差异(P<0.05),且>40岁时,各组牙根根管壁增龄性变化更明显(P<0.05);男性上颌第一前磨牙颊根腭侧壁厚度总体高于女性,有显著差异(P<0.05);随着距离根尖越近,男性与女性近颊根腭侧管壁厚度均减小,各组之间均有统计学差异(P<0.05)。男性腭侧壁最薄处为(0.58±0.11)mm,女性为(0.50±0.10)mm,二者有显著差异(P<0.05)。结论:上颌第一前磨牙颊根腭侧凹陷的发生率较高,根管治疗时应采取小锥度根管锉清理根管;40岁以上时腭侧壁增龄性变化更明显。Objective:To investigate the symmetry of palatal wall thickness of the buccal root of maxillary first premolar and its relationship with gender,age,and different root positions using cone beam computed tomography(CBCT),and provide references for clinical diagnosis and treatment.Methods:CBCT data of 259 patients from our department were selected and divided into four groups according to age.The thickness of buccal and palatal walls of root canals at different root positions was measured and analyzed.Results:The incidence rate of palatal groove is 77.9%,the thickness of palatal walls was similar on both sides,and there was no significant difference(P>0.05).The thickness of palatal walls at different root positions increased with age,and there was a significant difference(P<0.05).In addition,the aging changes of root canal walls in each group were more obvious in patients over 40 years old(P<0.05).The thickness of palatal wall of maxillary first premolar in males was generally higher than that in females,and there was a significant difference(P<0.05).As the distance from the root apex decreased,the thickness of palatal and buccal walls of root canals in both males and females decreased,and there was a statistical difference between groups(P<0.05).The thinnest part of palatal wall in males was(0.58±0.11)mm,and in females it was(0.50±0.10)mm,with a significant difference(P<0.05).Conclusion:The occurrence rate of concavity on the buccal and palatal sides of the maxillary first premolar is relatively high.Small taper root canal files should be used for cleaning during root canal treatment.The aging changes of palatal walls were more obvious in patients over 40 years old.
