摘　　要：目的:探讨miR-139-5p在老年胃癌中的表达及其与患者临床病理特征和预后关系。方法:选取我院收治的38例老年胃癌患者(年龄>65岁)为研究对象,采用Real-time PCR法检测患者癌组织、淋巴结和癌旁正常胃组织中miR-139-5p、转录因子4(TCF4)表达水平。应用免疫组织化学法检测胃癌组织和癌旁正常胃组织中TCF4蛋白表达水平。分析比较miR-139-5p、TCF4表达水平与胃癌患者临床病理特征的关系。同时采用生物信息法分析TCGA数据库中胃癌患者的预后数据,比较TCF4高低表达组患者总生存(OS)和无疾病进展生存(DFS)是否存在差异。结果:正常胃组织和胃癌组织中miR-139-5p相对表达量分别为-3.06±1.43和-4.09±2.13,差异有统计学意义(P=0.016)。未转移淋巴结和转移淋巴结中miR-139-5p相对表达量分别为0.25±1.64和-1.59±1.67,差异有统计学意义(P=0.001)。转录因子4(TCF4)基因3’-UTR区域ACUGUAC序列与miR-139-5p 5’-UTR区域UGACAUC结合紧密。提示TCF4可能为miR-139-5p的靶基因。Real-time PCR结果显示,TCF4基因在胃癌组织和转移淋巴结组织中的表达水平显著高于对应的癌旁组织(P=0.036)和未转移淋巴结组织(P=0.027)。且癌组织中TCF4的表达水平与miR-139-5p呈负相关(r_(pearson)=-0.42,P=0.03)。免疫组化显示TCF4蛋白主要表达于细胞核和细胞浆,38例患者癌组织中TCF4蛋白阳性26例(68.4%),阴性12例(31.6%)。miR-139-5p表达水平与胃癌患者淋巴结转移(P<0.05)和肿瘤病理分级(P<0.05)有关。TCF4表达水平与胃癌患者淋巴结转移(P<0.05)和肿瘤浸润深度(P<0.05)有关。Log-rank检验显示,TCF4高表达组患者总生存(OS)显著低于TCF4低表达患者(HR=1.4,P=0.032),而高低表达组患者无疾病进展生存(DFS)差异无统计学意义(HR=1.4,P=0.094)。结论:miR-139-5p在老年胃癌患者中低表达,并可能通过靶向调控TCF4抑制肿瘤的进展。TCF4高表达与胃癌患者总生存期减低有关,并有望成为胃癌Objective:To investigate the expression of miR-139-5p in elderly gastric cancer and its relationship with clinical pathological characteristics and prognosis of patients.Methods:38 elderly gastric cancer patients(over 65 years old)admitted to our hospital were selected as the research subjects.Real-time PCR was used to detect the expression levels of miR-139-5p and transcription factor 4(TCF4)in cancer tissue,lymph nodes,and normal gastric tissue adjacent to cancer.Immunohistochemical method was used to detect the expression level of TCF4 protein in gastric cancer tissue and adjacent normal gastric tissue.Analyze and compare the relationship between the expression levels of miR-139-5p and TCF4 and the clinical pathological characteristics of gastric cancer patients.At the same time,bioinformatics was used to analyze the prognosis data of gastric cancer patients in the TCGA database,and to compare whether there were differences in overall survival(OS)and disease progression free survival(DFS)between the high and low expression groups of TCF4.Results:The relative expression levels of miR-139-5p in normal gastric tissue and gastric cancer tissue were-3.06±1.43 and-4.09±2.13,respectively,with statistically significant differences(P=0.016).The relative expression levels of miR-139-5p in non-metastatic and metastatic lymph nodes were 0.25±1.64 and-1.59±1.67,respectively,with statistically significant differences(P=0.001).Transcription Factor 4(TCF4)Gene 3’-ACUGUAC sequence in UTR region and miR-139-5p 5’-UGACAUC in the UTR region is tightly integrated.It suggests that TCF4 may be a target gene for miR-139-5p.The Real-time PCR results showed that the expression level of TCF4 gene in gastric cancer tissue and metastatic lymph node tissue was significantly higher than that in corresponding adjacent cancer tissue(P=0.036)and non-metastatic lymph node tissue(P=0.027).The expression level of TCF4 in cancer tissue is negatively correlated with miR-139-5p(r_(pearson)=-0.42,P=0.03).Immunohistochemistry showed that TCF4
