血清肿瘤标志物检测对胃癌分期的评估作用

摘　　要：目的探讨血清肿瘤标志物检测对胃癌分期的评估作用。方法选取2020年3月至2021年10月本院收治的120例胃癌患者作为研究对象,患者均经病理组织活检确诊,根据TMN分期将其分Ⅳ期17例、Ⅲ期45例、Ⅱ期41例及Ⅰ期17例。患者均接受血清肿瘤标志物检测[血清癌胚抗原(CEA)、癌胚抗原19-9(CA19-9)、糖类抗原72-4(CA72-4)、糖类抗原CA125(CA125)],比较不同分期血清肿瘤标志物水平、阳性率,比较不同血清肿瘤标志物检测阳性率。结果胃癌Ⅰ期CA125、CA72-4、CA19-9、CEA水平均低于Ⅱ、Ⅲ、Ⅳ期,且Ⅱ期低于Ⅲ、Ⅳ期,Ⅲ期低于Ⅳ期,差异有统计学意义(P<0.05)。胃癌Ⅳ期CA125阳性率高于Ⅰ、Ⅱ、Ⅲ期,差异有统计学意义(P<0.05);胃癌Ⅳ、Ⅲ期联合检测阳性率高于Ⅰ、Ⅱ,Ⅱ期高于Ⅰ期,差异有统计学意义(P<0.05);其余不同分期血清肿瘤标志物检测阳性率两两比较差异无统计学意义。CA125、CA72-4、CA19-9、CEA及联合检测阳性率比较差异有统计学意义(P<0.05);联合检测阳性率高于CA125、CA72-4、CA19-9、CEA,CEA阳性率高于CA72-4、CA19-9,CA19-9阳性率低于CA125,差异有统计学意义(P<0.05)。结论血清肿瘤标志物与胃癌分期密切相关,临床可通过监测多项血清肿瘤标志物联合检测评估胃癌。Objective To explore the evaluation of the staging of gastric cancer by detection of serum tumor markers.Methods 120 patients with gastric cancer admitted from March 2020 to October 2021 were selected as the study objects.All patients were confirmed by pathological biopsy and were divided into stageⅣwas 17 cases,stageⅢwas 45 cases,stageⅡwas 41 cases and stageⅠwas 17 cases according to TMN staging.All patients received serum tumor markers detection(carcinoembryonic antigen[CEA],carcinoembryonic antigen 19-9[CA19-9],carbohydrate antigen 72-4[CA72-4],carbohydrate antigen 125[CA125]),the levels and positive rates of serum tumor markers in different stages were compared,and the positive rates of different serum tumor markers were compared.Results The levels of CA125,CA72-4,CA19-9 and CEA in gastric cancer stageⅠwere lower than those in stageⅡ,ⅢandⅣ,and those in stageⅡwere lower than those in stageⅢandⅣ,and those in stageⅢwere lower than those in stageⅣ,and the differences were statistically significant(P<0.05).The positive rate of CA125 in gastric cancer stageⅣwas higher than that in stageⅠ,ⅡandⅢ,and the differences were statistically significant(P<0.05);the positive rate of combined detection in stageⅣandⅢof gastric cancer was higher than that in stageⅠandⅡ,and that in stageⅡwas higher than that in stageⅠ,and the difference was statistically significant(P<0.05);there was no significant difference in the positive rate of other serum tumor markers in different stages.There were significant differences in the positive rates of CA125,CA72-4,CA19-9,CEA and combined detection(P<0.05).The positive rate of combined detection was higher than that of CA125,CA72-4,CA19-9 and CEA,the positive rate of CEA was higher than that of CA72-4 and CA19-9,and the positive rate of CA19-9 was lower than that of CA125,and the differences were statistically significant(P<0.05).Conclusion Serum tumor markers are closely related to the stage of gastric cancer,and the combined detection of multiple s
