摘　　要：目的:探讨超敏C反应蛋白和淋巴细胞趋化因子在急性淋巴细胞白血病中的表达水平及临床意义。方法:选取2018年5月—2020年5月河南科技大学第一附属医院收治的100例急性淋巴细胞白血病患者作为观察组,另选取同期100例健康体检者作为对照组,根据不同预后结局将急性淋巴细胞白血病患者分为缓解组67例和未缓解组33例。比较观察组和对照组、缓解组和未缓解组血清超敏C反应蛋白和淋巴细胞趋化因子水平,应用ROC曲线分析血清超敏C反应蛋白和淋巴细胞趋化因子患者预后的判断价值。结果:观察组血清超敏C反应蛋白和淋巴细胞趋化因子水平分别为(21.34±7.95)mg/L、(38.62±10.79)pg/mL,均高于对照组的(4.86±2.17)mg/L、(7.91±1.94)pg/mL,差异有统计学意义(t=19.998、28.012,P<0.05);缓解组血清超敏C反应蛋白和淋巴细胞趋化因子水平分别为(19.47±4.15)mg/L、(24.68±5.13)pg/mL,均低于未缓解组的(34.58±4.79)mg/L、(42.51±12.74)pg/mL,差异有统计学意义(t=23.841、12.982,P<0.05);血清超敏C反应蛋白[AUC:72.70,SE:0.053,Sig:<0.001,95%CI:(0.623,0.832)]和淋巴细胞趋化因子[AUC:76.30,SE:0.061,Sig:<0.001,95%CI:(0.643,0.883)]对急性淋巴细胞白血病患者预后预测价值低于联合检测预测价值[AUC:89.80,SE:0.043,Sig:<0.001,95%CI:(0.815,0.982)]。结论:急性淋巴细胞白血病患者血清超敏C反应蛋白和淋巴细胞趋化因子水平较健康者高,与患者预后密切相关,二者联合检测评估患者的预后具有较好的预测价值。Objective:To investigate the expression levels of ultrasensitive C-reactive protein and lymphocyte chemokines in acute lymphoblastic leukemia and their clinical significance.Methods:100 cases of acute lymphoblastic leukemia patients admitted to the hospital from May 2018 to May 2020 were selected as the observation group,and another 100 cases of healthy medical checkups in the same period were selected as the control group,and the patients with acute lymphoblastic leukemia were divided into 67 cases in the remission group and 33 cases in the non-remission group according to different prognostic endings.The serum ultrasensitive C-reactive protein and lymphocyte chemokine levels were compared between the observation group and the control group,and between the remission group and the non-remission group,and the ROC curves were applied to analyze the prognostic value of the patients with serum ultrasensitive C-reactive protein and lymphocyte chemokine.Results:The serum ultrasensitive C-reactive protein and lymphocyte chemokine levels in the observation group were(21.34±7.95)mg/L and(38.62±10.79)pg/mL respectively,which were higher than(4.86±2.17)mg/L and(7.91±1.94)pg/mL in the control group,and the differences were statistically significant(t=19.998,28.012,P<0.05).The serum ultrasensitive C-reactive protein and lymphocyte chemokine levels in the remission group were(19.47±4.15)mg/L and(24.68±5.13)pg/mL respectively,which were lower than(34.58±4.79)mg/L and(42.51±12.74)pg/mL in the non-remission group,and the difference was statistically significant(t=23.841,12.982,P<0.05).The prognostic predictive value of serum ultrasensitive C-reactive proteinAUC:72.70,SE:0.053,Sig:<0.001,95%CI[(0.623,0.832)]and lymphocyte chemokines AUC:76.30,SE:0.061,Sig:<0.001,95%CI:[0.643,0.883]in patients with acute lymphoblastic leukemia was lower than the predictive value of the combined testAUC:89.80,SE:0.043,Sig:<0.001,95%CI:[0.815,0.982].Conclusion:Serum ultrasensitive C-reactive protein and lymphocyte chemokine levels are higher
