胃癌患者外周血PLR、HPR与血清CA724的变化及临床意义

摘　　要：目的探讨胃癌患者血小板/淋巴细胞(PLR)、血红蛋白/血小板(HPR)与血清糖链抗原724(CA724)的变化及临床意义。方法回顾性收集2016年11月至2020年11月就诊于武汉科技大学附属孝感医院的106例胃癌组、125例胃息肉组和132名健康对照组的临床资料。比较3组的外周血PLR、HPR、血清CA724水平,分析外周血PLR、HPR与血清CA724的相关性及其与患者病理特征的关系;比较不同临床分期患者的外周血PLR、HPR、血清CA724水平;采用受试者工作特征(ROC)曲线分析外周血PLR、HPR和血清CA724单独及联合检测对胃癌的诊断效能。结果胃癌组患者的外周血PLR值为169.50(109.36,215.10),明显高于胃息肉组[140.61(133.06,148.16)]及健康对照组[116.56(90.72,138.11)],外周血HPR值为0.57(0.53,0.62),明显低于胃息肉组[0.67(0.59,0.84)]及健康对照组[0.70(0.58,0.79)],差异均有统计学意义(P<0.05);而胃息肉与健康对照组外周血PLR值、HPR比较,差异均无统计学意义(P>0.05)。胃癌组患者血清CA724水平为2.38(1.40,4.57)IU/mL,明显高于健康对照组[1.69(1.02,2.94)IU/mL],差异有统计学意义(P<0.05),但与胃息肉组[2.01(0.92,3.58)IU/mL]比较,差异无统计学意义(P>0.05)。胃癌组患者的外周血PLR与血清CA724呈正相关(r=0.460,P<0.001);外周血HPR与血清CA724无相关性(r=-0.163,P=0.094)。不同T分期、淋巴结转移、临床分期胃癌患者的外周血PLR、HPR水平比较,差异均有统计学意义(P<0.05)。外周血PLR随着胃癌临床TNM分期等级升高而增高,外周血HPR随胃癌临床TNM分期等级升高而降低,血清CA724水平与胃癌的临床TNM分期无关。单指标检测时,外周血HPR对胃癌的诊断效能优于外周血PLR、血清CA724。多指标联合检测时,血清CA724+外周血HPR联合检测比血清CA724单独检测具有更大AUC值,且具有更好的特异度,具有最佳的诊断效能。结论与健康体检者比较,胃癌患者外周血HPR水平降低,外周血PLR、血清CA724�Objective To investigate the changes and clinical significance of platelet/lymphocyte(PLR),hemoglobin/platelet(HPR),and serum carbohydrate antigen 724(CA724)in patients with gastric cancer.Methods The clinical data of 106 patients with gastric cancer(gastric cancer group),125 patients with gastric polyp(gastric polyp group)and 132 healthy subjects(healthy control group)admitted to Xiaogan Hospital Affiliated to Wuhan University of Science and Technology from November 2016 to November 2020 were retrospectively collected.The levels of PLR,HPR and CA724 in the three groups were compared to analyze the correlation between PLR,HPR and CA724 and the relationship between PLR,HPR and pathological features of patients.PLR,HPR and CA724 levels in patients with different clinical stages were compared.The diagnostic efficacy of PLR,HPR and CA724 alone or in combination was analyzed by receiver operating characteristic(ROC)curve.Results The PLR value of the gastric cancer group was 169.50(109.36,215.10),which was significantly higher than that of the gastric polyp group[140.61(133.06,148.16)]and the healthy control group[116.56(90.72,138.11)],and the HPR value was 0.57(0.53,0.62),which was significantly lower than that of the gastric polyp group[0.67(0.59,0.84)]and the healthy control group[0.70(0.58,0.79)],the differences were statistically significant(P<0.05),however,there was no statistically significant difference in PLR and HPR between the gastric polyp group and the healthy control group(P>0.05).The level of serum CA724 in the gastric cancer group was 2.38(1.40,4.57)IU/mL,which was significantly higher than that in the healthy control group[1.69(1.02,2.94)IU/mL],the difference was statistically significant(P<0.05),however,there was no statistically significant difference in serum CA724 levels between the gastric cancer group and the gastric polyp group[2.01(0.92,3.58)IU/mL](P>0.05).PLR in gastric cancer group was positively correlated with CA724(r=0.460,P<0.001),there was no correlation between HPR and CA724(r=-0.163,P=
