摘 要:目的比较超声生物显微镜(UBM)下急性闭角型青光眼(AACG)与慢性闭角型青光眼(CACG)眼部生物学参数差异性,进一步分析两种闭角型青光眼发作的可能解剖危险因素。方法回顾性选取2020年1月至2023年1月河南省安阳市眼科医院收治的35例单眼发作的AACG患者(发作眼和未发作眼各35只),以及35例双眼不对称损伤的CACG患者(损伤较重眼和损伤较轻眼各35只)为研究对象。使用UBM对所有患者进行检查,获取中央角膜厚度(CCT)、中央前房深度(ACD)、前房宽度(ACW)、750μm处的房角开放距离(AOD_(750))、晶状体拱高(LV)、小梁网-睫状突距离(TCPD)、虹膜厚度750μm(IT_(750))、瞳孔直径(PD)、小梁网-睫状突夹角(TCA)、虹膜根部附着位置(IRI)、虹膜形态;使用光学生物测量仪IOL Master700测量所有患者双眼轴长(AXL)。结果AACG发作眼组的CCT、LV大于CACG损伤较重眼组,ACD、AOD_(750)、TCPD和TCA小于CACG损伤较重眼组(P<0.05);AACG未发作眼组的LV大于CACG损伤较轻眼组,ACD、TCPD、TCA小于CACG损伤较轻眼组(P<0.05);AACG发作眼组的CCT、LV和PD大于AACG未发作眼组,ACD、AOD_(750)和TCPD小于AACG未发作眼组(P<0.05);CACG损伤较重眼组的ACD、TCPD小于CACG损伤较轻眼组(P<0.05)。AACG组IRI 3种类型较均衡,CACG组较集中于靠前型和中间型;AACG组虹膜形态主要集中于膨隆型,CACG组两种类型较均衡。结论ACD、LV和TCA可能是AACG区别于CACG的主要解剖结构参数。Objective To compare the difference of ocular biological parameters between acute angle-closure glaucoma(AACG)and chronic angle-closure glaucoma(CACG)under ultrasound biomicroscopy(UBM),and further analyze the possible anatomical risk factors for the onset of two types of angle-closure glaucoma.Methods A total of 35 patients(35 eyes with attack and 35 eyes without attack)with monocular AACG and 35 CACG patients(35 eyes with severe damage and 35 eyes with mild damage)with bilateral asymmetric injury treated in Anyang Eye Hospital of Henan Province from January 2020 to January 2023 were retrospectively selected as study objects.All patients were examined by UBM,and central corneal thickness(CCT),anterior chamber depth(ACD),anterior chamber width(ACW),angle opening distance at 750μm(AOD_(750)),lens vault(LV),trabecular meshwork-ciliary process distance(TCPD),iris thickness at 750μm(IT_(750)),pupil diameter(PD),trabecular-ciliary process angle(TCA),iris insertion(IRI)and iris morphology were obtained;the axial length(AXL)was measured in all patients by the optical biometric instrument IOL Master700.Results The CCT and LV of the AACG group with attack eye were greater than those of the CACG group with severe eye damage,and the ACD,AOD_(750),TCPD and TCA were smaller than those of the CACG group with severe eye damage(P<0.05);the LV of the AACG group without attack eye was greater than that of the CACG group with mild eye damage,and the ACD,TCPD and TCA were smaller than those of the CACG group with mild eye damage(P<0.05);the CCT,LV and PD in the AACG group with attack eye were greater than those in the AACG group without attack eye,and the ACD,AOD_(750) and TCPD were smaller than those in the AACG group without attack eye(P<0.05);the ACD and TCPD in the CACG group with severe eye damage were smaller than those in the CACG group with mild eye damage(P<0.05).The 3 types of IRI in the AACG group were more balanced,while those in the CACG group were more concentrated in front type and middle type;the iris morphology of
