摘 要:目的探讨高频彩色多普勒超声鉴别诊断乳腺影像报告和数据系统(BI-RADS)4a类乳腺纤维腺瘤与乳腺癌的价值,旨在为临床诊断提供参考。方法回顾性分析2021年5月至2022年5月经病理检查结果证实的249例患者(包括BI-RADS 4a类乳腺纤维腺瘤98例,乳腺癌151例)的超声征象,评估超声定性参数对BI-RADS 4a类乳腺纤维腺瘤与乳腺癌的诊断效能。结果BI-RADS 4a类乳腺纤维腺瘤与乳腺癌患者的年龄及病灶大小、最大径、纵横比、形态、边缘、包膜、后方回声、彩色多普勒血流成像技术(CDFI)血流信号分级及阻力指数(RI)比较,差异具有统计学意义(P<0.05);BI-RADS 4a类乳腺纤维腺瘤与乳腺癌患者的病灶方位、回声及钙化情况比较,差异无统计学意义(P>0.05)。病灶超声特征中边缘的曲线下面积(AUC)最大,为0.899(95%CI 0.843~0.962),其次为CDFI血流信号分级、年龄,分别为0.861(95%CI 0.794~0.929)、0.783(95%CI 0.698~0.867),三者的灵敏度分别为95.8%、90.3%,86.1%,特异度分别为71.4%、32.7%、57.1%。结论高频彩色多普勒超声为鉴别诊断BI-RADS 4a类乳腺纤维腺瘤与乳腺癌提供了重要信息,具有一定的诊断价值,值得临床推广应用。Objective To investigate the value of high frequency color Doppler ultrasound in differential diagnosis of breast imaging reporting and date system(BI-RADS)4a breast fibronenoma and breast cancer,in order to provide reference for clinical diagnosis.Methods Ultrasound signs of 249 patients confirmed by pathological examination from May 2021 to May 2022(including 98 cases of BI-RADS 4a breast fibroadenoma and 151 cases of breast cancer)were retrospectively analyzed,and the diagnostic efficacy of ultrasound qualitative parameters for BI-RADS 4a breast fibroadenoma and breast cancer was evaluated.Results There were statistically significant differences in age,lesion size,maximum diameter,aspect ratio,shape,edge,envelope,posterior echo,color Doppler flow imaging(CDFI)blood signal grade and resistance index(RI)between BI-RADS 4a breast fibroadenoma and breast cancer patients(P<0.05);there were no significant differences in lesion orientation,echo and calcification between BI-RADS 4a breast fibroadenoma and breast cancer patients(P>0.05).The area under curve(AUC)of the lesion edge was the highest in ultrasound features,with values of 0.899(95%CI 0.843-0.962),followed by CDFI blood signal grade and age,with values of 0.861(95%CI 0.794-0.929)and 0.783(95%CI 0.698-0.867),respectively;the sensitivity of the three were 95.8%,90.3%and 86.1%,with specificity of 71.4%,32.7%and 57.1%,respectively.Conclusion High frequency color Doppler ultrasound provides important information for the differential diagnosis of BI-RADS 4a breast fibroadenoma and breast cancer,and has certain diagnostic value,which is worthy of clinical promotion and application.
