首发抑郁障碍患者自杀意念和行为与血脂水平、临床特征的关系

摘　　要：目的:探讨首发抑郁障碍患者自杀意念和行为与血脂水平、临床特征的关系。方法:纳入自2016年2月至2021年5月于本院精神科住院治疗的首发抑郁障碍患者429例。根据病史中有无自杀意念或(和)自杀行为分为无自杀意念无自杀行为组(A组228例)、有自杀意念无自杀行为组(B组147例)、有自杀意念和行为组(C组54例),对三组人口学资料、汉密尔顿抑郁量表24项(HAMD-24)评分、血脂水平[总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)]进行统计分析。结果:①年龄:C组(26.67±10.57)岁<B组(35.00±13.29)岁<A组(39.97±12.61)岁(P均<0.05);C组HAMD-24总分及认知障碍因子分均高于A组、B组(P均<0.05);②以年龄为协变量,TC:C组(3.24±0.17 mmol/L)<B组(4.10±0.07 mmol/L)<A组(4.33±0.06 mmol/L)(P均<0.05);③Logistic回归显示年龄、TC水平、LDL水平是首发抑郁障碍患者出现自杀意念和行为的影响因素。结论:有自杀意念和行为的首发抑郁障碍患者病情相对更严重;年龄偏小、TC水平低及LDL水平高可能是首发抑郁障碍患者发生自杀意念和行为的危险因素。Objective:To investigate the relationship between suicidal ideation and behavior and blood lipid levels and clinical characteristics in patients with first-episode depressive disorder.Method:429 inpatients with first-episode depression disorder who received treatment in the psychiatric department in the Second Affiliated Hospital of Kunming Medical University from February 2016 to May 2021 were recruited.they were divided into three groups according to the history of the presence of suicidal ideation or/and suicide behavior:the group without suicidal ideation or action(group A,228 cases),the group with suicidal ideation but no action(group B,147 cases),the group with suicidal ideation and action(group C,54 cases).Their demographic data,Hamilton Depression Scale-24 item(HAMD-24)score,blood lipid levels total cholesterol(TG)and triglyceride(TC),low density lipoprotein(LDL),high-density lipoprotein(HDL)were analyzed.Results:①average age:C group(26.67±10.57)<B group(35.00±13.29)<A group(39.97±12.61)(all P<0.05);The HAMD-24 total score of group C and recognitive obstacles factor score were higher than group A and group B(all P<0.05);②With age as covariate,TC:group C(3.24±0.17 mmol/L)<group B(4.10±0.07 mmol/L)<group A(4.33±0.06 mmol/L)(all P<0.05);③Logistic regression showed that age,the TC,LDL levels were influencial factors of suicidal ideation and behaviors for patients with first-episode depressive disorder.Conclusion:Patients with first-episode depressive disorder who had suicidal ideation and behavior are relatively more severely ill;younger age,lower TC levels,and higher LDL levels may be risk factors for the development of suicidal ideation and behavior in patients with first-episode depressive disorder.
