摘　　要：目的对比观察脉络膜脱离型视网膜脱离首次治疗时应用玻璃体切割术(PPV)、巩膜扣带术(SB)的效果,探讨脉络膜脱离型视网膜脱离首次手术方案的选择。方法脉络膜脱离型视网膜脱离患者22例(22眼),根据手术方式不同分为PPV组(11例11眼)、SB组(11例11眼)。PPV组第一次手术时接受PPV治疗,SB组第一次手术时接受SB治疗,第一次手术后视患者情况实施二次或多次手术治疗。记录并比较两组患者第一次手术后视网膜复位率和最终视网膜复位率、手术后三个月时的最终矫正视力及手术中并发症发生率、手术后并发症发生率。结果PPV组第一次手术后视网膜复位率为81.8%(9/11)、最终视网膜复位率为81.8%(9/11),SB组第一次手术后视网膜复位率为63.6%(7/11)、最终视网膜复位率为90.9%(10/11),两组相比,P均>0.05。两组患者术后三个月时的最终矫正视力、手术中并发症发生率、手术后并发症发生率相比,P均>0.05。结论PPV、SB治疗脉络膜脱离型视网膜脱离的效果相当。Objective To compare and observe the efficacy of pars plana vitrectomy(PPV)and scleral buckling(SB)in the initial treatment of rhegmatogenous retinal detachment with choroidal detachment and to explore the choice of the initial surgical approach for such cases.Methods Twenty-two patients(22 eyes)with rhegmatogenous retinal de⁃tachment and choroidal detachment were included.They were divided into two groups:the PPV group(11 patients,11 eyes)and the SB group(11 patients,11 eyes).In the PPV group,patients received PPV as the initial treatment,while in the SB group,patients underwent SB as the initial treatment.Subsequent surgeries were performed based on individual pa⁃tients'conditions.The rates of retinal reattachment after the first surgery,final retinal reattachment,final corrected visual acuity three months postoperatively,and the incidence of intraoperative and postoperative complications were recorded and compared between the two groups.Results In the PPV group,the retinal reattachment rate after the first surgery was 81.8%(9/11),and the final retinal reattachment rate was 81.8%(9/11).In the SB group,the retinal reattachment rate after the first surgery was 63.6%(7/11),and the final retinal reattachment rate was 90.9%(10/11).There was no signif⁃icant difference between the two groups(all P>0.05).At three months after surgery,there were no significant differences between the two groups in terms of final corrected visual acuity,the incidence of intraoperative complications,or the inci⁃dence of postoperative complications(all P>0.05).Conclusion In the treatment of rhegmatogenous retinal detachment with choroidal detachment,PPV and SB demonstrated comparable efficacy.
