摘 要:目的比较分析机器人辅助下单侧经皮后凸成形术(percutaneous kyphoplasty,PKP)手术治疗骨质疏松性椎体压缩骨折的临床疗效。方法选取2019年1月~2022年6月接受机器人辅助下单侧PKP手术治疗的71例骨质疏松性椎体压缩性骨折(osteoporotic vertebral compression fracture,OVCF)患者作为研究对象(机器人PKP组),同时选取22例未行机器人辅助操作、术者徒手穿刺的OVCF患者作为对照组(传统PKP组),观察患者VAS评分及椎体骨水泥填充超过中线占比的指标。结果术前VAS评分相比较,机器人PKP组与传统PKP组患者术后VAS评分均显著降低,差异有统计学意义(P<0.05);两组术后1 d、术后1个月的VAS评分比较,差异无统计学意义(P>0.05)。机器人PKP组各年龄段的患者的骨水泥填充超过中线占比百分率比较,差异均无统计学意义(P>0.05);但机器人PKP组骨水泥填充超过中线占比百分率显著高于传统PKP组(P<0.05)。结论机器人辅助下单侧PKP手术对于老年骨质疏松性椎体压缩骨折患者具有良好的疗效,各年龄段患者的骨水泥分布均优于传统PKP手术且无明显差异性,可作为OVCF患者的首选手术方案。Objective To compare and analyze the clinical efficacy of unilateral robot-assisted PKP in the treatment of osteoporotic vertebral compression fractures.Methods A total of 71 patients with osteoporotic vertebral compression fracture who underwent unilateral robot-assisted PKP surgery from January 2019 to June 2022 were selected as study subjects.At the same time,22 patients with OVCF who did not undergo robot-assisted operation and who underwent free-hand puncture were selected as the control group to observe the VAS score and the index of vertebral cement filling exceeding the midline.Results Compared with the preoperative VAS score in the robotic PKP group and the traditional PKP group,the postoperative VAS score was significantly reduced in PKP of the robot-assisted and the free-hand puncture,with significant statistical significance(P<0.05).There was no significant difference in VAS scores of 1 day and 1 month after surgery between the two groups(P>0.05).Thecomparison of the percentage of bone cement filling exceeding the median line in patients of all ages in the robot PKP group had no significant statistical significance(P>0.05),while the percentage of bone cement filling exceeding the median line in the robot PKP group had significant statistical significance compared with that in the control group(traditional PKP group)(P<0.05).Conclusion Unilateral robot-assisted PKP surgery has a good effect on elderly patients with osteoporotic fractures.The surgical efficacy of all age groups is much better than that of the traditional PKP group with no significant difference,and it can be used as the first choice for patients with OVCF.
