玻璃体视网膜手术后高眼压的临床分析

摘　　要：目的 对玻璃体视网膜术后高眼压原因及其临床处理进行分析。方法 对2020年5月—2022年5月收治的行玻璃体视网膜手术的96例患者(96眼)临床资料进行回顾分析,分析其术后高眼压发生原因,并采取药物或者手术治疗处理。结果 本组患者中,术后24h内有53例发病,24~48h有33例发病,48~72h有10例发病。患者的年龄与眼内科手术史是术后早期高眼压引发的主要危险因素,原发疾病、糖皮质激素、全视网膜光凝术均为早期高眼压引发的次要危险因素。96眼术后第1d高眼压的发生率是35.42%(34/96),术后1周内发生高眼压的发生率是12.50%(12/96);术后2~4周内高眼压的发生率是5.21%(5/96)。不同情况下进行玻璃体切割术之后高眼压的发生率有所不同。结论 在玻璃体视网膜手术后引发高眼压的因素比较多,术前及术中加强部分可控因素预防,术后加强眼压监测,做到早发现、早干预,能有效控制高眼压。Objective To analyze causes and clinical management of high intraocular pressure after vitreoretinal surgery.Methods The paper reviewed and analyzed clinical data of 96 patients(96 eyes)with vitreoretinal surgery from May 2020 to May 2022,analyzed causes of postoperative high intraocular pressure,and carried on medication or surgical treatment.Results Tthere were 53 cases of onset within 24 hours after surgery,33 cases within 24-48 hours,and 10 cases within 48-72 hours.Age and history of ophthalmic surgery of patients were main risk factors of early postoperative high intraocular pressure,while primary disease,glucocorticoids,and whole retinal photocoagulation were secondary risk factors of early postoperative high intraocular pressure.Incidence of high intraocular pressure on the first day after surgery of 96 eyes was 35.42%(34/96),and incidence of high intraocular pressure within 1 week after surgery was 12.50%(12/96).Incidence of high intraocular pressure within 2-4 weeks after surgery was 5.21%(5/96).Incidence of high intraocular pressure after vitrectomy varied in different situations.Conclusion There are many factors causing high intraocular pressure after vitreoretinal surgery.It is necessary to carry on preoperative and intraoperative prevention of some controllable factors,strengthen postoperative monitoring of intraocular pressure to achieve early detection and intervention,which can control high intraocular pressure effectively.
