摘 要:目的 探究MBT直丝弓矫治器联合微型种植体支抗(MIA)矫治对安氏Ⅱ类1分类错畸形下颌骨硬组织、软组织及牙周健康的影响。方法 回顾性分析四川大学华西广安医院安氏Ⅱ类1分类错畸形患者资料,采用倾向性匹配评分法分为两组,传统口外弓支抗(HGA)组和MIA组,其中HGA组采用MBT直丝弓矫治器联合HGA矫治,MIA组采用MBT直丝弓矫治器联合MIA矫治,排除年龄、性别等混杂因素后,最终获得47对匹配样本,比较两组临床治疗指标,治疗前、后两组下颌骨硬组织、软组织头影测量数据、美容效果、牙周健康情况及不良反应。结果 MIA组平均矫治时间、拔牙间隙关闭时间、咬合打开时间、排齐整平时间、磨牙位移时间均短于HGA组(P<0.05);治疗后,两组ANB、U1-SN、U1-NA、ULP、LLP均降低,L1-MP、L1-NB、Z角升高(P<0.05),且治疗后组间比较差异有统计学意义(P<0.05),两组治疗前后SNA、SNB、MP-SN、MP-FH、Ulip-E、Llip-E比较差异无统计学意义(P>0.05);MIA组整齐美观度、舒适度、固位能力均高于HGA组(P<0.05);治疗后,两组GI、PLI、SBI值均提升(P<0.05),且MIA组高于HGA组(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 安氏Ⅱ类1分类错畸形采用MBT直丝弓和MIA联合矫治疗效显著,可明显缩短患者治疗周期,美容效果良好,且更利于维护牙周健康。Objective To explore the influence of MBT straight wire appliance combined with micro-implant anchorage(MIA)on the hard tissue and soft tissue of mandible and periodontal health in Class II Division 1 malocclusion.Methods The data of patients with Class II Division 1 malocclusion in West China Guang'an Hospital of Sichuan University were retrospectively analyzed.The patients were divided into two groups.The headgear anchorage(HGA)group was treated with MBT straight wire appliance combined with traditional HGA appliance,while the MIA group was treated with MBT straight wire appliance combined with MIA appliance.The propensity score matching method was used for grouping to eliminate confounding factors such as age and gender,and 47 pairs of matched samples were finally obtained.The clinical treatment indicators,indicators of the hard tissue and soft tissue of mandible and periodontal health before and after treatment,cosmetic effect,and adverse reactions were compared between the two groups.Results The mean correction time,extraction space closing time,occlusal opening time,alignment time and molar displacement in MIA group were significantly shorter than those in HGA group(P<0.05).After treatment,the AB plane angle(ANB),included angle between long axis of upper central incisor and sella nasion plane(U1-SN),upper incisor-nasion subspinale connectio(U1-NA),upper lip protrusion(ULP)and lower lip protrusion(LLP)were significantly decreased while the lower incisor-mandibular plane(L1-MP),lower incisor-nasion supramental(L1-NB)and included angle between facial profile and eyeear plane(Z)were significantly increased in both groups(P<0.05),and the differences between the two groups were statistically significant(P<0.05).There was no significant difference in sella nasion A point(SNA),sella nasion B point(SNB),mandibular plane-sella nasion(MP-SN),mandibular plane-frankfort horizontal plane(MP-FH),distance from upper lip to E line(Ulip-E)and distance from lower lip to E-line(Llip-E)between the two groups before and after tr
