不同矫治器在安氏Ⅱ类牙性错[牙合]畸形治疗中的应用比较

摘　　要：目的比较不同矫治器在安氏Ⅱ类牙性错[牙合]畸形治疗中的应用效果。方法选取2018年1月至2020年9月于抚州市临川区第一人民医院接受牙齿矫治的80例安氏Ⅱ类牙性错[牙合]畸形患者作为研究对象,按照使用的矫治器不同分为A组(固定矫治器)和B组(无托槽隐形矫治器),各40例。比较两组矫治效果[美国正畸学会客观评分系统(ABO-OGS)]、矫治前后牙周健康指数[菌斑指数(PLI)、牙龈指数(GI)、牙周探诊深度(PD)]、头影测量指标[上中切牙长轴与鼻根点-上齿槽座点连线的交角(U1-NA)、上中切牙长轴与前颅底平面交角(UI-SN)、下颌平面与前颅底平面交角(MP-SN)、上唇凸点至E线距离(UL-EP)、下中切牙牙轴与下颌平角的交角(LI-MP)、下唇凸点至E线距离(LL-EP)]。结果两组牙齿排列、边缘嵴高度差、颊舌向倾斜度、咬合关系、覆盖、咬合接触、牙根平行度、邻牙接触评分比较差异无统计学意义。矫治1年,两组GI、PLI评分均高于矫治前,PD大于矫治前,但B组GI、PLI评分均低于A组,PD小于A组,差异有统计学意义(P<0.05)。矫治1年,两组U1-NA、UI-SN、LI-MP均大于矫治前,UL-EP、LL-EP均长于矫治前,但B组U1-NA、UI-SN、LI-MP均小于A组,UL-EP、LL-EP均短于A组,差异有统计学意义(P<0.05);矫治1年,两组MP-SN比较差异无统计学意义。结论固定矫治器与无托槽隐形矫治器应用于安氏Ⅱ类牙性错[牙合]畸形患者中的矫治效果相当,但无托槽隐形矫治器对前牙唇倾度控制及侧貌维持更有优势,且更利于保持患者牙周健康,值得临床推广应用。Objective To compare the application effects of different appliances in the treatment of ClassⅡmalocclusion.Methods A total of 80 patients with ClassⅡmalocclusion who received dental correction in the First People's Hospital of Linchuan District,Fuzhou City from January 2018 to September 2020 were selected as the research subjects,they were divided into the group A(fixed appliance)and the group B(bracketless in-visible appliance)according to the different appliances used,with 40 cases in each group.The orthodontic effect(American Orthodontic Association Objective Scoring System[ABO-OGS]),periodontal health index before and after correction(plaque index[PLI],gingival index[GI],probing depth[PD]),cephalometric indicators before and after correction(upper incisor-nasion-ploint A[U1-NA],upper incisor-sella nasion plan[UI-SN],mandib-ular plane-sella nasion plan[MP-SN],distance from upper lip to E line[UL-EP],the angle between the axis of the lower central incisor and the man-dibular plane[LI-MP],distance from lower lip to E line[LL-EP])were compared between the two groups.Results There were no significant differ-ences in tooth arrangement,marginal ridge height difference,buccolingual inclination,occlusal relationship,overjet,occlusal contact,root parallel-ism and adjacent tooth contact score between the two groups.After 1 year of correction,the scores of GI and PLI of the two groups were higher than those before treatment,and PD was higher than that before treatment,but the scores of GI and PLI in the group B were lower than those in the group A,and PD was smaller than that in the group A,the differences were statistically significant(P<0.05).After 1 year of treatment,the U1-NA,UI-SN and LI-MP of the two groups were higher than those before treatment,UL-EP and LL-EP were longer than those before treatment,but the U1-NA,UI-SN and LI-MP in the group B were lower than those in the group A,UL-EP and LL-EP were shorter than those in the group A,the differences were statistically significant(P<0.05);after 1 year of corr
