高渗点眼联合局部给氧治疗白内障术后重度角膜水肿的疗效及预后分析

摘　　要：目的探讨高渗点眼联合局部给氧治疗白内障术后重度角膜水肿的临床价值。方法选取2018年1月至2019年1月枣庄市峄城区人民医院收治的90例白内障术后重度角膜水肿患者作为研究对象,采用随机数字表法分为对照组与研究组,每组45例。对照组接受常规滴眼药物治疗,研究组在对照组治疗基础上联合高渗点眼与局部给氧治疗。比较两组治疗前后角膜荧光染色(CFS)、最佳矫正视力(BCVA)、眼表疾病指数量表(OSDI)、临床疗效及不良反应发生情况。结果治疗前,两组BCVA、CFS评分、OSDI评分比较差异无统计学意义,对照组CFS评分治疗前后比较差异无统计学意义;治疗后,两组BCVA均高于治疗前,OSDI评分均低于治疗前,研究组CFS评分低于治疗前和对照组,BCVA高于对照组,OSDI评分低于对照组,差异有统计学意义(P<0.05)。研究组治疗优良率为95.56%,高于对照组的82.22%,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论高渗点眼联合局部给氧治疗白内障术后重度角膜水肿疗效确切,可有效改善患者角膜水肿症状,提高患者视物能力。Objective To investigate the clinical value of hypertonic glucose combined with local oxygen therapy in the treatment of severe cor-neal edema after cataract surgery.Methods A total of 90 patients with severe corneal edema after cataract surgery treated in Yicheng District People's Hospital from January 2018 to January 2019 were selected as the study subjects,and they were divided into the control group and the study group by random number table method,with 45 cases in each group.The control group received conventional eye drop drug treatment,and the study group com-bined hypertonic glucose combined with local oxygen therapy on the basis of the control group.The corneal fluorescence staining(CFS),best correct-ed visual acuity(BCVA),ocular surface disease index(OSDI)before and after treatment,clinical efficacy and adverse reactions were compared be-tween the two groups.Results Before treatment,there were no significant differences in BCVA,CFS score and OSDI score between the two groups,and there was no significant difference in CFS score between the control group before and after treatment;after treatment,the BCVA of the two groups were higher than before treatment,OSDI score were lower than before treatment,CFS score in study group was lower than before treatment and control group,BCVA was higher than control group,OSDI score was lower than control group,the differences were statistically significant(P<0.05).The rate of excellent and good treatment in the study group was 95.56%,which was higher than 82.22%in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Hy-pertonic glucose combined with local oxygen therapy is effective in treating severe corneal edema after cataract operation,which can effectively im-prove the symptoms of corneal edema and improve the visual ability of patients.
