和胃清化汤联合兰索拉唑胶囊治疗慢性非萎缩性胃炎脾胃湿热证的疗效及对胃动力、胃黏膜分泌水平的影响

摘　　要：目的观察和胃清化汤联合兰索拉唑胶囊治疗慢性非萎缩性胃炎(CNG)脾胃湿热证的疗效及对胃动力、胃黏膜分泌水平的影响。方法将180例CNG脾胃湿热证患者按照随机数字表法分为2组,对照组90例予兰索拉唑胶囊治疗,治疗组90例在对照组治疗基础上加和胃清化汤治疗。2组均治疗2周。比较2组疗效;比较2组治疗前后中医证候评分变化;比较2组治疗前后胃泌素(GAS)、胃动素(MTL)、生长抑素(SS)水平变化;比较2组治疗前后白细胞介素6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)水平变化;比较2组不良反应情况。结果治疗组总有效率95.56%(86/90),对照组总有效率84.44%(76/90),治疗组疗效优于对照组(P<0.05)。2组治疗后各项中医证候评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后GAS、SS水平较本组治疗前升高(P<0.05),MTL水平较本组治疗前降低(P<0.05);治疗后治疗组GAS、SS水平均高于对照组(P<0.05),MTL水平低于对照组(P<0.05)。2组治疗后IL-6、CRP、TNF-α水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。结论和胃清化汤联合兰索拉唑胶囊治疗CNG脾胃湿热证,可有效改善患者临床症状和胃动力,减轻胃黏膜损伤,降低炎症水平。Objective To investigate the efficacy of Hewei Qinghua Decoction combined with lansoprazole capsules in the treatment of chronic non-atrophic gastritis(CNG)ofspleen-stomach damp-heat syndrome,and the influence on gastric motility and gastric mucosal secretion level.Methods A total of 180 patients with CNG of spleen-stomach damp-heat syndrome were randomly allocated in control group(n=90)and treatment group(n=90).Based on lansoprazole capsules,and patients of the treatment group were additionally treated with Hewei Qinghua Decoction.The treatment was conducted for 2 weeks in the both groups,aiming to compare the curative effects,and the TCM syndrome scores,the levels of gastrin(GAS),motilin(MTL)and somatostatin(SS),interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α),as well as adverse reactions and recurrence between groups before and after treatment.Results The total clinical response rate in the treatment group was significantly higher than that in the control group(95.56%[86/90]vs 84.44%[76/90],P<0.05).The each dimension score and total score of TCM syndrome were significantly decreased in the two groups(P<0.05),the decrease in the treatment group was more common(P<0.05).Significantly increased GAS and SS and significantly decreased MTL were detected in the two groups(all P<0.05),the improvement was more in the treatment group(all P<0.05).After treatment,the levels of IL-6,CRP and TNF-αin the two groups were significantly decreased when compared with those before treatment(P<0.05),and the decrease in the treatment group was more significant(P<0.05).Conclusion For patients with CNG of spleen-stomach damp-heat syndrome,Hewei Qinghua Decoction combined with lansoprazole capsules can effectively improve the patients’clinical symptoms and gastric motility,relieve gastric mucosal damage,and lower inflammatory level.
