摘 要:目的研究分析超声超微血管成像技术(SMI)在乳腺良恶性肿瘤的微血管构筑中的诊断应用情况。方法回顾性分析2022年3月至2023年3月本院收治的女性乳腺肿瘤患者共60例作为研究对象,所有患者均行超声造影微血管成像技术(MVI)检查和超声超微血管成像技术(SMI)检查。结果以活检或者手术后病理检查结果作为最终判定标准,结果显示,60个病灶中良性病灶37个,恶性病灶23个,良性病灶中21个为纤维腺瘤,8个为导管内乳头状瘤,7个为纤维囊性乳腺病,1个为慢性乳腺炎;恶性病灶中,18个为浸润性导管瘤,2个为导管原位癌,2个为浸润性乳头状癌,1个为浸润性小叶癌。MVI检查发现,良性病灶主要以无血管型、线型、树枝型这3种类型多见,病灶内部的血管粗细无差异,分布相对均匀,而恶性病灶主要以蟹足型为主,周围可见扭曲粗大的血管,而残根型在良性或恶性病灶中均可出现;SMI检查发现,良性病灶仍是以树枝型,线型和无血管型为主,而恶性病灶则以蟹足型、残根型更为多见;两种检查方法的敏感性、特异性、准确率无明显差异(P>0.05)。结论超声超微血管成像技术在诊断乳腺良恶性肿瘤的效能与超声造影微血管成像无明显差异,且前者不需注射造影剂,为无创检查,成本更低,并且超微血管成像技术可有效显示病灶内微小血管以及低速血流,可更全面细致的观察病灶的血管构筑情况,适合临床推广应用。Objective To study and analyze the application of ultrasound microvascular imaging(SMI)in the diagnosis of microvascular architecture of breast benign and malignant tumors.Methods From March 2022 to March 2023,a retrospective analysis of 60 female patients with breast cancer admitted in our hospital,all patients were examined by contrast-enhanced ultrasound microvascular imaging(MVI)and ultrasound microvascular imaging(SMI).Results Using biopsy or postoperative pathological examination as the final criterion,the results showed that 37 of 60 lesions were benign,23 were malignant,and 21 of 60 lesions were fibroadenoma,among the malignant lesions,18 were invasive ductal tumors,2 were ductal carcinoma in situ,and 2 were invasive papillary carcinoma,one for Invasive lobular carcinoma.The results of MVI showed that the most common types of benign lesions were non-vascular,linear and dendritic.The blood vessels in the lesions had no difference in size and were relatively uniform in distribution,while the most common type of malignant lesions was crab foot,the twisted and thick blood vessels were found around the tumor,and the residual root type could be found in both benign and malignant lesions,there was no significant difference in sensitivity,specificity and accuracy between the two methods(P>0.05).Conclusion There is no significant difference between ultrasound and contrast-enhanced microangiography in the diagnosis of breast tumors,and the former is a non-invasive method without injection of contrast media,the micro-vessel and low-velocity blood flow can be displayed effectively by the technique of ultra-micro-vessel imaging,and the vascular architecture of the lesion can be observed more comprehensively and carefully,which is suitable for clinical application.
