双眼有晶状体眼后房型人工晶状体植入术后拱高相关性的研究

摘　　要：目的分析具有相似眼前节特征的双眼有晶状体眼后房型人工晶状体(ICL)植入术后拱高的相关性。方法回顾性病例系列研究。分析山西省眼科医院2022年3月至9月双眼ICL植入术81例(162眼)的临床资料,术前记录患者的前房深度、睫状沟水平直径(hSTS)、术前等效球镜度和自然晶状体厚度,同时记录术中植入的ICL尺寸并计算水平压力,使用前段光学相干断层扫描成像(AS-OCT)测量术眼术后1 d、1周及1个月的拱高,通过相关性分析确定影响双眼拱高差异的因素。结果术后1个月,双眼拱高差值为18.0(-43.5,79.5)μm,拱高差值绝对值为71.0(28.5,105.5)μm。双眼拱高差与水平压力呈正相关(r=0.25,P=0.023),与前房深度、hSTS、术前等效球镜度、自然晶状体厚度、ICL尺寸和拱高无相关性(均P>0.05)。结论具有相似眼前节特征的双眼ICL植入术后拱高相似,但也可能存在相当大的差异,这反映了拱高的可变性。Objective To analyze the differences in vault size between fellow eyes with similar anterior segment biometry.Methods This was a retrospective case series study.The study included 162 eyes of 81 patients who underwent implantable collamer lens(ICL)implantation in Shanxi Eye Hospital from Mar.to Sep.2022.Preoperative anterior chamber depth,horizontal sulcus-to-sulcus diameter(hSTS),ICL spherical equivalent,lens thickness,interoperative ICL size and the horizontal pressure(ICL size-hSTS)were recorded.Vaults were measured with anterior segment optical coherence tomography at 1 d,1 week and 1 month postoperatively.Bivariate correlation was applied to seek for factors affecting vault inter-eye differences.Results At 1 month postoperatively,mean vault inter-eye difference was 18.0(-43.5,79.5)μm,and the mean absolute vault inter-eye differences was 71.0(28.5,105.5)μm.Vault inter-eye differences were positively correlated with the level of horizontal compression(r=0.25,P=0.023)and negatively correlated with anterior chamber depth,hSTS,preoperative spherical equivalent,lens thickness,ICL size,or vault(all P>0.05).Conclusion Vaults measured in fellow eyes may present similar size,but there is some degree of baseline variability in the vault.
