摘　　要：目的 探讨宫颈癌并盆腔/腹膜后淋巴结转移患者的同步加量容积旋转调强方案放射治疗作用。方法 选择2020年1月~2021年8月在本院腹部肿瘤科收治的初次行根治性放疗子宫颈癌患者50例,均开展同步放化疗模式治疗。放疗方案为外照射(同步加量调强模式)结合腔内近距离照射,外照射设定子宫、宫颈、阴道、宫旁组织、淋巴结引流区以及阳性淋巴结部位作为治疗靶区,参数设定:PGTVnd:55.9~59.8Gy/2.15~2.3Gy/26f;PTV:46.8~48.1Gy/1.8~1.85Gy/26 f。结果 疗效评价:CR (47/50) 94.0%,PR (3/50) 6.0%。Ⅲ度和Ⅳ度骨髓抑制发生率(11/50) 22.0%,Ⅲ度肠道反应发生率(2/50) 4%。结论 宫颈癌并盆腔/腹膜后淋巴结转移患者给予同步加量容积旋转调强方案放射治疗,在整体治疗效果比较理想,大部分患者可以耐受该治疗方案,并取得令人满意的结果。Objective Toinvestigate the effect of simultaneous enhanced volume rotation intensive-modulated radiotherapy in patients with cervical cancer complicated pelvic/retroperitoneal lymph node metastasis.Method Fifty patients with cervical cancer who underwent initial radical radiotherapy for the first time in the department of Abdominal Oncology ofour hospital from January 2020 to August 2021 were selected allof them were underwent synchronous radiotherapy and chemotherapy.The radiotherapy regimen consisted ofextemal irradiation(synchronous dosing and intensity modulation mode)combined with intracavitary brachytherapy.Uterus,cervix,vagina,paruterine tissues,lymph node drainage area and positive lymph node sites were set as treatment targets for external irradiation,and parameters were set as follows:PGTVnd:55.9-59.8Gy/2.15-2.3Gy/26f;PTV:46.8~48.1Gy/1.8~1.85Gy/26 f.Resul ts Efficacy evaluation:CR(47/50)94.0%,PR(3/50)6.0%.The incidence of grade II and IV myelosuppression(11/50)was 22.0%,and the incidence of grade II intestinal reaction(2/50)was 4%.Conclusion Patients with cervical cancer complicated with pelvic/retroperitoneal lymph node metastasis were treated with synchronous volume rotation intensity modulation radiotherapy,which had a good overall therapeutic effect.Most patients could tolerate this therapy and achieved satisfactory results.
