摘 要:目的:探讨单光子发射计算机断层(SPECT)扫描评价不同剂量 131I治疗分化型甲状腺癌对患者唾液腺功能的影响。 方法:本研究采用自身对照研究设计,选取2022年1—12月北部战区总医院核医学科收治的68例首次行 131I治疗的分化型甲状腺癌患者,男31例,女37例,年龄(42.1±7.2)岁,年龄范围为33~52岁,按照 131I治疗剂量将患者分为低剂量组(≤100 mCi, n=42)与高剂量组(>100 mCi, n=26),分别于 131I治疗前1 d、 131I治疗后3个月行唾液腺SPECT显像,比较低剂量组与高剂量组患者 131I治疗前后双侧腮腺、双侧颌下腺的最大横截面积、摄取百分比和维生素C酸刺激后排泌百分比功能参数的变化情况。 结果:高剂量组治疗后3个月左侧腮腺[(19.6±3.1)cm^(2)]及右侧腮腺[(19.4±2.5)cm^(2)]最大横截面积低于治疗前[(23.6±2.4)cm^(2)、(23.5±2.4)cm^(2)];低剂量组患者治疗后3个月左侧腮腺[(69.4±15.7)%]、右侧腮腺[(61.3±19.1)%]、左侧颌下腺[(52.1±13.7)%]、右侧颌下腺[(55.2±12.7)%]的摄取百分比均低于治疗前[(82.1±14.2)%、(83.3±16.6)%、(69.5±11.3)%、(67.1±10.4)%];高剂量组患者治疗后3个月左侧腮腺[(63.9±12.5)%]、右侧腮腺[(62.1±18.2)%]、左侧颌下腺[(55.1±12.5)%]、右侧颌下腺[(51.2±16.7)%]的摄取百分比均低于治疗前[(81.7±12.2)%、(86.2±14.3)%、(71.2±13.3)%、(69.3±12.4)%];低剂量组患者治疗后3个月左侧腮腺[(49.4±13.2)%]、右侧腮腺[(48.1±12.7)%]、左侧颌下腺[(50.1±13.5)%]、右侧颌下腺[(55.6±13.9)%]的酸刺激后排泌百分比均低于治疗前[(61.2±11.2)%、(60.2±11.4)%、(79.2±12.6)%、(82.1±14.4)%];高剂量组患者治疗后3个月左侧腮腺[(43.9±13.1)%]、右侧腮腺[(41.1±13.7)%]、左侧颌下腺[(50.3±10.6)%]、右侧颌下腺[(49.8±12.9)%]的酸刺激后排泌百分比均低于治疗前[(63.1±10.2)%、(59.6±12.5)%、(73.6±14.2)%、(76.6±15.1)%],差异均有统计学意义( P<0.05)。 结论:131I治疗对唾液腺功能可�ObjectiveTo explore single photon emission computed tomography(SPECT)to evaluate the effects of different doses of 131I on the function of salivary glands in patients with differentiated thyroid cancer.MethodsThis study uses a self-controlled study design,selected 68 patients with differentiated thyroid cancer treated with 131I in the first time in the department of Nuclear Medicine in Northern Theater General Hospital from January to December 2022,including 31 males and 37 females,aged(42.1±7.2)years old,ranging form 33 to 52 years old.Patients were divided into low-dose group(≤100 mCi,n=42)and the high-dose group(>100 mCi,n=26)at the 131I treatment dose,SPECT imaging of the salivary glands was performed 1 d before 131I treatment and 3 months after 131I treatment,to compare the maximum cross-sectional area,uptake percentage,and vitamin C acid stimulated functional parameters of the bilateral parotid and bilateral submandibular glands before and after treatment with 131I in the low-dose group and high-dose group.ResultsIn the high-dose group at 3 months after treatment,the maximum cross-sectional area of the left parotid gland[(19.6±3.1)cm^(2)]and the gland[(19.4±2.5)cm^(2)]in the right parotid were lower than before treatment[(23.6±2.4)cm^(2),(23.5±2.4)cm^(2)],in the low-dose group at 3 months after treatment,the uptake percentage of left parotid gland[(69.4±15.7)%],right parotid gland[(61.3±19.1)%],left submandibular gland[(52.1±13.7)%]and right submandibular gland[(55.2±12.7)%]were all lower than before treatment[(82.1±14.2)%,(83.3±16.6)%,(69.5±11.3)%,(67.1±10.4)%],in the high-dose group at 3 months after treatment,the uptake percentage of the left parotid gland[(63.9±12.5)%],right parotid gland[(62.1±18.2)%],left submandibular gland[(55.1±12.5)%],right submandibular gland[(51.2±16.7)%]were all lower than before treatment[(81.7±12.2)%,(86.2±14.3)%,(71.2±13.3)%,(69.3±12.4)%],in the low-dose group at 3 months after treatment,the percentage of left parotid gland[(49.4±13.2)%],right parotid
