摘  要:目的评价T1 mapping在腮腺肿瘤鉴别诊断中的价值。方法回顾性分析145例治疗前行T1 mapping检查并经病理证实的腮腺肿瘤患者,其中101例为良性肿瘤(66例多形性腺瘤,18例Warthin瘤,11例基底细胞腺瘤和6例其他肿瘤),44例为恶性肿瘤。测量肿瘤及对侧正常腺体增强前T1值(T1pre)、增强后T1值(T1post),计算相对值rT1pre、rT1post及增强前后T1差异(ΔTl)、T1值变化率,比较T1pre、T1post、ΔT1、T1值变化率、rT1pre及rT1post组间差异,并绘制受试者工作特征曲线。结果腮腺良性肿瘤的ΔT1显著高于恶性肿瘤(P=0.014),ΔT1鉴别腮腺良、恶性肿瘤的曲线下面积为0.621。多形性腺瘤与Warthin瘤、多形性腺瘤与恶性肿瘤、Warthin瘤与基底细胞腺瘤、Warthin瘤与恶性肿瘤之间T1pre、ΔT1、T1值变化率及rT1pre的差异均有统计学意义(均P<0.01)。基底细胞腺瘤与恶性肿瘤之间T1pre、ΔT1及rT1pre的差异均有统计学意义(均P<0.05)。T1pre、ΔT1、T1值变化率及rT1pre鉴别多形性腺瘤与Warthin瘤、多形性腺瘤与恶性肿瘤、Warthin瘤与基底细胞腺瘤、Warthin瘤与恶性肿瘤的曲线下面积分别为0.936、0.957、0.935和0.860,0.675、0.733、0.729和0.660,0.919、0.919、0.884和0.879,0.872、0.903、0.870和0.759。T1pre、ΔT1及rT1pre鉴别基底细胞腺瘤与恶性肿瘤的曲线下面积分别为0.725、0.750和0.686。结论T1 mapping有助于腮腺肿瘤的鉴别。Objective To evaluate the value of TI mapping in differential diagnosis of parotid gland tumors.Methods 145 patients with pathologically proven parotid gland tumors underwent T1 mapping before therapy were retrospectively analyzed,including 101 cases of benign tumors(66 cases of pleomorphic adenomas,18 cases of Warthin tumors,11 cases basal cell adenomas and 6 cases of other tumors),and 44 cases of malignant tumors.The T1 values(T1 pre and T1 post)of tumors and contralateral glands were obtained.The relative TIpre(rT1pre),relative TIpost(rT1 post),T1 difference before and after enhancement(△TI)and TI rate of change were calculated.TIpr,TI st TI,TI rate of change,rT1 pre and rTI pos between groups of parotid tumors were compared,and receiver operating characteristic curves was drawn.ResulstsATI in the benign parotid tumor were significantly higher than those in the malignant tumor(P=0.014),the AUC of△T1 for differential diagnosis of benign and malignant parotid tumors was 0.621.Comparison between pleomorphic adenoma and Warthin tumor,pleomorphic adenoma and malignant tumor,Warthin tumor and basal cell adenoma,and Warthin tumor and malignant tumor showed all statistically different in TIpre,△T1,T1 rate of change and rTIpre(all P<O.O1).Comparison between basal cell adenoma and malignant tumor showed all statistically different in TIpr:,ATI,and rTI pre(all P<0.05).The AUC of TIpre,T1,TI rate of change and rTIpre for differentiating pleomorphic adenoma from Warthin tumor were O.9360.957,0.935 and 0.860,respectively.The AUC of TI pre,△T1,T1 rate of change and rTIpre for differentiating pleomorphic adenoma from malignant tumor were 0.675,0.733,0.729 and 0.660,respectively.The AUC of TIpre,△T1,T1 rate of change and rT1pre for differentiating Warthin tumor from basal cell adenoma were 0.919,0.919,0.884 and 0.879,respectively.The AUC of TI pre,TI,TI rate of change and rTI pre for differentiating Warthin tumor from malignant tumor were 0.872,0.903,0.870 and 0.759,respectively.The AUC of T1 pre,AT1,and rT1 pre for
