摘　　要：甲胎蛋白阳性胃癌(alpha-fetoprotein-producing gastric carcinoma,AFPGC)是一种特殊类型的胃癌,与普通胃癌相比,其恶性程度高、易发生转移且预后差。针对AFPGC的诊疗面临多方面的挑战,鉴别诊断较为困难。目前AFPGC的诊断方式包括检测血清甲胎蛋白(alpha-fetoprotein,AFP)水平及病理染色。尽管传统的胃癌治疗方式部分适用于AFPGC,但是在改善预后方面效果不佳,针对AFPGC的精准治疗策略可以提高疗效。本文综述了目前AFPGC的诊断和治疗进展,以明确目前AFPGC的研究现状,为AFPGC的早发现、早诊断以及临床治疗提供思路。Alpha-fetoprotein-positive gastric cancer is a special type of gastric cancer,which is highly malignant,prone to metastasis,and has a poor prognosis compared with ordinary gastric cancer.Diagnosing and treatment of AFPGC face many challenges,and the differential diagnosis is difficult.Currently,the diagnosis of AFPGC includes the detection of serum alpha-fetoprotein level and pathologic staining.Although traditional gastric cancer treatments are partially applicable to AFPGC,they are ineffective in improving the prognosis,and precision treatment strategies for AFPGC can improve efficacy.In this paper,we summarize the current research progress in the diagnosis and treatment of AFPGC,so as to clarify the current research status and provide ideas for the early detection,early diagnosis and clinical treatment of AFPGC.
