血清TK1、CEA和CA19-9检测对胃癌的诊断价值

摘　　要：目的研究血清细胞质胸苷激酶1(thymidine kinase 1,TK1)、癌胚抗原(carcinoembryonic antigen,CEA)和糖类抗原19-9(carbohydrate antigen 19-9,CA19-9)检测对胃癌的诊断价值。方法选取2022年6月—2023年5月福建省邵武市立医院内胃癌患者(n=95,观察组)、胃部良性病变患者(n=100,对照1组)与健康对象(n=100,对照2组)进行比较研究。三组均行血清TK1、CEA和CA19-9检测,比较检测结果差异。而后对不同病理特征胃癌患者的检测结果行比较分析及Spearman相关性分析,评价三者检测对胃癌的诊断价值。结果观察组血清TK1、CEA、CA19-9检测结果高于对照1组、对照2组,差异有统计学意义(P<0.05);不同肿瘤直径、临床分期、分化程度以及淋巴结转移情况胃癌患者的血清TK1、CEA、CA19-9检测结果比较,差异有统计学意义(P<0.05);血清TK1、CEA、CA19-9检测结果与胃癌患者的肿瘤直径、临床分期、淋巴结转移均呈正相关(P<0.05),与胃癌患者的分化程度呈负相关(P<0.05);血清TK1、CEA、CA19-9三者联合检测对胃癌的诊断价值最高(AUC=0.925,P<0.05)。结论血清TK1、CEA、CA19-9联合检测对胃癌有较高的诊断价值,对鉴别患者病情发展、病理特征亦有积极作用。Objective To study the diagnostic value of serum thymidine kinase 1(TK1),carcinoembryonic antigen(CEA)and carbohydrate antigen 19-9(CA19-9)in gastric cancer.Methods Patients with gastric cancer(n=95,observation group),patients with benign gastric lesions(n=100,control 1 group)and healthy subjects(n=100,control 2 group)in Shaowu Municipal Hospital,Fujian Province were selected to conduct a comparative study from June 2022 to May 2023.All three groups were tested for serum TK1,CEA,and CA19-9,and the differences in the test results were compared.Then,comparative analysis and Spearman correlation analysis were conducted on the detection results of gastric cancer patients with different pathological characteristics to evaluate the diagnostic value of the three tests for gastric cancer.Results The detection results of serum TK1,CEA,and CA19-9 in the observation group were higher than those in the control group 1 and control group 2,with statistical significance(P<0.05).The comparison of serum TK1,CEA,and CA19-9 detection results among gastric cancer patients with different tumor diameters,clinical stages,differentiation levels,and lymph node metastasis showed statistically significant differences(P<0.05).The detection results of serum TK1,CEA,and CA19-9 were positively correlated with lesion diameter,disease stage,and lymph node metastasis in gastric cancer patients(P<0.05),but negatively correlated with the degree of differentiation in gastric cancer patients(P<0.05).The combined detection of serum TK1,CEA,and CA19-9 has the highest diagnostic value for gastric cancer(AUC=0.925,P<0.05).Conclusion The combined detection of serum TK1,CEA,and CA19-9 has high diagnostic value for gastric cancer,and has a positive role in distinguishing the progression and pathological characteristics of patients.
