静息态功能MRI在首诊精神分裂症患者中的应用价值分析

摘　　要：目的:探讨静息态功能MRI在首诊精神分裂症患者中的应用价值。方法:选取2018年1月—2020年12月在首都医科大学附属北京安定医院登记有效的110例精神分裂症患者,采用静息态功能MRI和CT对患者进行诊断,对患者的异常状况进行记录,分析不同诊断方式的导电图异常检出结果,比较检出准确率,分析静息态功能MRI功能连接异常脑区状态。结果:静息态功能MRI对精神分裂症患者诊断准确率98.18%高于CT检查的86.36%,差异有统计学意义(P<0.05);静息态功能MRI诊断单发病灶发生率高于CT,但差异无统计学意义(P>0.05);精神分裂症患者静息态功能MRI对比CT,结果表明,精神病患者可见明显的左侧楔前叶、扣带回、左侧中央前回、双侧中央后回、双侧舌回、右侧颞中回、右侧颞上极、左侧颞横回、双侧顶上小叶、左侧岛叶功能连接异常,且增强;左侧额中回、左侧顶下小叶、双侧额上回、右侧眶下回功能接连异常,且减弱。结论:静息态功能MRI在精神分裂症患者的临床诊断中准确度较高,临床应用价值显著,可推广应用。Objective To investigate the application value of resting-state functional MRI in the first diagnosis of schizophrenia.Methods A total of 110 schizophrenic patients effectively registered in Beijing Anding Hospital affiliated to Capital Medical University from January 2018 to December 2020 were selected.The patients were diagnosed by resting state functional MRI and CT,the abnormal conditions of the patients were recorded,the abnormal detection results of conductive graphs of different diagnostic methods were analyzed,and the detection accuracy rate was compared.The resting state functional MRI was used to analyze the abnormal state of functional connections in the brain.Results The diagnostic accuracy of fMRI in patients with schizophrenia was 98.18%higher than that of CT(86.36%),and the difference was statistically significant(P<0.05).The diagnosis of single lesion by resting-state fMRI was higher than that of CT,but the difference was not statistically significant(P>0.05).The results of restingstate functional fMRI compared with CT showed that the functional connections of the left anterior cuneus,cingulate gyrus,left anterior central gyrus,bilateral posterior central gyrus,bilateral lingual gyrus,right middle temporal gyrus,right superior temporal pole,left transverse temporal gyrus,bilateral parietal lobules and left insula were significantly abnormal and enhanced in the patients with psychosis.The left medial frontal gyrus,left inferior parietal lobule,bilateral superior frontal gyrus,and right infraorbital gyrus were successively abnormal and weakened.Conclusion The diagnostic accuracy of resting-state functional MRI in the clinical diagnosis of schizophrenia patients is high,the clinical application value is significant,and it can be popularized.
