孕期血清17种氨基酸含量与新生儿体质量的影响因素分析

摘  要:目的分析孕期血清17种氨基酸含量与新生儿体质量的影响因素。方法随机选取2020年2月—2022年2月在广西南宁市第一人民医院埌东医院立卡定期产检的200例单胎孕妇作为研究对象,产检时均采集静脉血并记录产检时体质量,进行17种氨基酸检测,整个孕期检测次数≥4次,取平均值,待孕妇分娩后,记录新生儿出生体质量,根据新生儿出生体质量分为低出生体重儿组(19例)、体质量正常组(164例)、巨大儿组(17例),通过Logistic回归分析,分析新生儿出生体质量的影响因素。结果3组孕妇天冬氨酸、蛋氨酸、瓜氨酸、色氨酸、酪氨酸、丝氨酸、天冬酰胺、异亮氨酸、精氨酸、组氨酸、鸟氨酸、亮氨酸、苏氨酸、丙氨酸、谷氨酰胺等氨基酸水平对比,差异无统计学意义(P>0.05),低出生体重儿组、体重正常组孕妇的苯丙氨酸水平(65.7±2.4)μmol/L、(64.8±2.2)μmol/L显著高于巨大儿组(52.1±2.14)μmol/L,谷氨酸水平(23.4±1.8)μmol/L、(24.7±2.5)μmol/L著低于巨大儿组(30.2±2.6)μmol/L,差异有统计学意义(P<0.05);Logistic回归分析显示,苯丙氨酸、谷氨酸水平高是导致新生儿出生体质量异常的重要影响因素(OR=1.043、1.005)。结论孕期血清17种氨基酸中苯丙氨酸、谷氨酸含量高是导致新生儿出生体质量异常的重要影响因素。Objective To analyze the influence factors of 17 amino acids in serum during pregnancy and neonatal weight.Methods A total of two hundred singleton pregnant women who received regular prenatal check-up at Nanning First People's Hospital Langdong Hospital from February 2020 to February 2022 were randomly selected as the study objects.Venous blood was collected and weight was recorded during delivery.17 kinds of amino acids were detected.The number of detection times in the whole pregnancy was≥4 times,and the average value was taken.After delivery,the neonatal weight was recorded and divided into low birth weight group(19 cases),normal weight group(164 cases)and giant baby group(17 cases).The influencing factors of birth weight were analyzed by Logistic regression analysis.Results There were no statistically significant differences in the levels of amino acids such as aspartic acld,methionine,citrulline,tryptophan,tyrosine,serine,aspartamide,isoleucine,arginine,histidine,ornithine,leucine,threonine,alanine and glutamine among there groups of pregnant women(P>0.05).The phenylalanine levels(65.7±2.4)μmol/L and(64.8±2.2)μmol/L in pregnant women in the low birth weight group and normal weight group were significantly higher than those in the macrosomia group(52.1±2.14)μmol/L,and the difference was statistically significant(P<0.05).Logistic regression analysis showed that high levels of phenylalanine and glutamate were important factors leading to abnormal birth weight(OR=1.043,1.005).Conclusion The high content of phenylalanine and glutamic acid in serum of 17 kinds of amino acids during pregnancy is an important factor that leads to abnormal neonatal weight.
