13例嗅神经母细胞瘤患者的MRI影像与病理特征对照分析

摘　　要：目的 探讨嗅神经母细胞瘤的多参数MRI影像学特点,提高该病诊断水平。方法 收集13例经病理学确诊嗅神经母细胞瘤患者的临床及MRI资料,分析多参数MRI征象。结果 13例患者中A期0例、B期2例、C期9例、D期2例,2例D期患者分别呈大脑镰旁和颈部淋巴结转移。13例患者的MRI均表现为肿瘤中心位于鼻腔顶部,主要侵犯筛窦12例、眼眶7例、前颅窝和额叶10例,呈典型“哑铃状”或“蘑菇状”改变9例。瘤体主要呈等/稍长T1、稍长/长T2信号,增强后13例均中度或明显强化,8例信号不均匀。结论 嗅神经母细胞瘤的多参数MRI影像具有特征性,可为该病的临床诊断提供准确而有价值的重要信息。Objective To investigate multi-parameter MRI imaging features of olfactory neuroblastoma,improve the diagnosis of this disease.Methods Clinical and multi-parameter MRI data of 13 patients with olfactory neuroblastoma diagnosed by pathology were collected and the multi-parameter MRI signs were analyzed.Results Among the 13 patients,there were 0 patients in stage A,2 patients in stage B,9 patients in stage C,2 patients in stage D,and 2 patients in stage D,respectively,with lymph node metastasis beside the falx of the brain and neck.MRI of all 13 patients showed that the tumor center was located at the top of the nasal cavity,mainly invaded ethmoid sinus in 12 cases,orbit in 7 cases,anterior cranial fossa and frontal lobe in 10 cases,and presented typical“dumbbell-shaped”or“mushroom-shaped”changes in 9 cases.The tumor mainly presented equal/slightly longer T1 signal and slightly longer/long T2 signal.After enhancement,13 cases showed moderate or obvious enhancement,and 8 cases showed uneven signal.Conclusions The multi-parameter MRI images of olfactory neuroblastoma are characteristic and can provide accurate and valuable information for the clinical diagnosis of this disease.
