摘  要:国内外研究表明核心自我评价会对临床护士的共情疲劳产生作用,但对二者之间关系机制的研究却鲜有报道.为探究临床护士核心自我评价与共情疲劳的关系,提出了一个有调节机制的中介模型,重点讨论了自我概念清晰度的中介作用,并考察职称的调节作用.选取3所综合医院261名临床护士作为研究对象,采用问卷法对临床护士核心自我评价、共情疲劳和自我概念清晰度进行调查.研究结果表明:1)共情疲劳与核心自我评价以及自我概念清晰度存在显著的负向相关关系,而核心自我评价与自我概念清晰度之间正相关性显著,检验结果P<0.01.2)自我概念清晰度对共情疲劳存在显著的负向回归影响,β=-0.256,显著性检验结果P=0.001,核心自我评价对共情疲劳存在显著的负向回归影响,β=-0.204,显著性检验结果P=0.008,而核心自我评价对自我概念清晰度存在显著的正向回归影响,β=0.345,显著性检验结果P<0.001,因此可以认为中介模型成立,且为部分中介效应.3)核心自我评价×职称交互项对共情疲劳存在显著的负向回归影响,P=0.001,对自我概念清晰度存在显著的正向回归影响,P<0.001.而自我概念清晰度×职称对共情疲劳存在显著的负向回归影响,P<0.001,因此职称因素在中介模型3条路径中均存在显著的调节作用.4)随着职称的提高,核心自我评价对自我概念清晰度的正向影响关系以及核心自我评价、自我概念清晰度对共情疲劳的负向影响关系均得到明显增强.5)综合比较各职称条件下中介效应量大小,对于主管护师及以上的被试来说,中介效应量为-0.671,且95%置信区间[-0.957,-0.481]不包含0,即随着职称的提高,中介效应量逐渐增大,有调节的中介效应模型成立.6)临床护士核心自我评价影响个体共情疲劳水平,这种影响被自我概念清晰度部分中介,且职称在二者关系之间起调节作用.Domestic and foreign studies have shown that core self-evaluation can affect the empathy fatigue of clinical nurses,but there are few reports on the mechanism of the relationship between the two.To explore the relationship between core self-evaluation and compassion fatigue in clinical nurses,a moderated mediation model is propose,focusing on the mediating role of self-concept clarity and the mediating role of the professional titles.A total of 261 clinical nurses from three general hospitals were selected as the research objects.The core self-evaluation,empathy fatigue and self-concept clarity of clinical nurses were investigated by questionnaire.The results show that:1)There is a significant negative correlation between empathy fatigue and core self-evaluation and self-concept clarity,while there is a significant positive correlation between core self-evaluation and self-concept clarity,the test result P<0.01.2)Self-concept clarity has a significant negative regression effect on compassion fatigue,β=-0.256,significance test result P=0.001,core self-evaluation has a significant negative regression effect on compassion fatigue,β=-0.204,significance test result p=0.008,while core self-evaluation has a significant positive regression effect on self-concept clarity,β=0.345,significance test result P<0.001,so it can be considered that the mediating model is established and it is a partial mediating effect.3)The interaction of core self-evaluation×professional title has a significant negative regression effect on empathy fatigue,P=0.001,and a significant positive regression effect on self-concept clarity,P<0.001,while self-concept clarity×professional title has a significant negative regression effect on empathy fatigue,P<0.001,so the professional title factor has a significant moderating effect in the three paths of the intermediary model.4)With the improvement of professional title,the positive influence of core self-evaluation on self-concept clarity and the negative influence of core self-evaluation and self-
