摘　　要：目的分析某中医医院糖尿病患者住院费用的影响因素,以期为合理控制糖尿病患者医疗费用增长和减轻患者经济负担提供参考。方法收集某三甲中医院2017年1月1日-2021年12月31日糖尿病患者住院信息,共收集有效数据5930例,在单因素分析、多元线性逐步回归分析的基础上,采用Apriori算法挖掘糖尿病患者住院费用与影响因素的关联关系。结果Apriori算法挖掘出4条有意义的关联规则,高药品费用、高中医医疗服务项目费用、高化验费和高检查费与高额费用存在强关联,4条规则中使得高额费用发生的可能性依次是原来的3.30、2.55、2.30和2.20倍;置信度分别是82.17%、63.45%、57.25%和54.76%;支持度分别为12.28%、11.21%、12.31%和12.12%;具体例数为728、665、730和719。结论住院天数、药品费用、耗材费用、检查费用均为影响糖尿住院费用的主要可控因素,在确保医疗质量的前提下,鼓励中医院保持中医特色,增加非药物疗法的比重,缩短无效住院天数,合理用药,规范耗材的使用,加强监管,优化收入结构,以减少患者的经济负担。Objectives To analyze the factors influencing the hospitalization expenses of diabetic patients in a TCM hospital,in order to provide reference for controlling the increase of medical expenses and reducing the economic burden of diabetic patients.Methods A total of 5930 cases of patients with diabetes were collected from a Three A and Tertiary TCM Hospital from January 1st,2017 to December 31st,2021,and valid data were collected.Based on univariate analysis and multiple linear stepwise regression analysis,the Apriori algorithm was used to explore the association between hospitalization costs and influencing factors of patients with diabetes.Results Apriori algorithm excavated four meaningful association rules.High drug cost,high TCM medical service cost,high laboratory cost and high inspection cost were strongly correlated with high cost.Among the four rules,the probability of high cost was 3.30,2.55,2.30 and 2.20 times of the original one.The confidence levels were 82.17%,63.45%,57.25%and 54.76%,respectively.The support degree was 12.28%,11.21%,12.31%and 12.12%,respectively.The specific numbers are 728,665,730 and 719.Conclusions Hospitalization days,drug costs,consumables costs,and inspection costs were the main controllable factors affecting the hospitalization costs of diabetes mellitus.On the premise of ensuring medical quality,Chinese medicine hospitals were encouraged to maintain the characteristics of traditional Chinese medicine,increase the proportion of non-drug therapy,shorten the number of ineffective hospitalization days,rationally use drugs,standardize the use of consumables,strengthen supervision,and optimize the income structure to reduce the economic burden of patients.
