口服曲安西龙治愈多发型婴幼儿血管瘤合并肝脏血管瘤1例

摘　　要：患儿女,33 d。出生3 d后患儿全身陆续出现红色肿物,并迅速增多、长大。皮肤科情况:头面、躯干、四肢可见百余个0.5~1.5 cm的红色肿物,界限清楚,充血明显,压之不退色。腹部彩超检查示:肝脏右叶可见一直径约1.5 cm的圆形低回声区,界限清楚,可探及动脉频谱,诊断为肝脏血管瘤。临床诊断为婴幼儿血管瘤(多发型,浅表型)合并肝脏血管瘤(局灶型)。口服曲安西龙片4 mg, 2次/d,连续口服20 d后复诊,患儿皮损消退60%以上。后改为口服曲安西龙片4 mg, 1次/d,连续使用30 d后复诊,患儿所有皮损完全消失,未遗留色素异常及瘢痕。彩超检查显示婴幼儿血管瘤及肝脏血管瘤均治愈。建议继续口服曲安西龙片1个月,随访半年未见复发。The patient was a 33-day-old female. Three days after birth, a red mass gradually appeared on the whole body, and increased rapidly. Dermatological examination found more than 100 red masses of 0.5-1.5 cm on the whole body, with clear boundaries and apparent congestion that did not fade under pressure. Abdominal color ultrasound examination showed a circular hypoechoic area with a diameter of about 1.5 cm in the right lobe of the liver, with clear boundaries and a clear arterial spectrum, which was diagnosed as hepatic hemangioma. The clinical diagnosis was infantile hemangioma(multi-type, superficial type)combined with hepatic hemangioma(focal type). Triancillon tablet 4 mg was given orally twice a day. After 20 days of continuous oral administration, the patient returned to the clinic. The skin lesions subsided by more than 60%. The treatment was changed to Trianxilong tablet 4 mg administered orally once a day. After 30 days, all skin lesions disappeared completely, and no pigment abnormalities or scars were left. Color ultrasound examination indicated that infantile hemangioma and liver hemangioma were cured. It was recommended to continue treatment with triancillon tablets for one more month, and no recurrence was observed during half a year's follow-up.
