摘  要:目的:探讨重度牙周炎伴冠心病患者外周血miR-146a、miR-155水平变化及临床意义。方法:选取32例重度牙周炎伴冠心病患者为A组,54例重度牙周炎患者为B组,45例健康体检者作为对照组,酶联免疫吸附法检测3组外周血的血清肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)和高密度脂蛋白(HDL),实时荧光定量PCR实验检测3组外周血miR-146a、miR-155,采用单因素方差分析影响重度牙周炎患者发生冠心病的因素,并对其影响因素进行Logistic回归分析,采用受试者工作曲线(ROC)分析外周血miR-146a、miR-155预测重度牙周炎患者发生冠心病的价值。结果:A组的miR-146a分别高于对照组、B组(P<0.05),A组的miR-155分别低于对照组和B组(P<0.05);A组的体重指数、血清CRP、miR-146a均高于B组(P<0.05),A组的受教育年限、miR-155均低于B组(P<0.05);Logistic多因素回归分析显示miR-146a是影响重度牙周炎患者发生冠心病的独立危险因素,miR-155是影响重度牙周炎患者发生冠心病的保护因素(P<0.05);ROC分析显示,miR-146a,miR-155预测重度牙周炎患者发生冠心病的最佳截断点分别为1.84和0.60,灵敏度分别为81.25%和84.37%,特异度分别为83.33%和72.22%。结论:重度牙周炎伴冠心病患者的miR-146a、miR-155均发生异常变化,miR-146a、miR-155可作为预测重度牙周炎患者发生冠心病的敏感指标,且联合预测价值更高。Objective:To explore the changes and clinical significance of miR⁃146a and miR⁃155 levels in peripheral blood of pa⁃tients with severe periodontitis and coronary heart disease.Methods:Thirty⁃two patients with severe periodontitis and coronary heart disease admitted to the hospital were selected as group A,another fifty⁃four patients with severe periodontitis were selected as group B,another fourty⁃five healthy individuals were selected as the control group.Enzyme⁃linked immunosorbent assay was used to detect labo⁃ratory indicators,including serum tumor necrosis factor⁃α(TNF⁃α),C⁃reactive protein(CRP),interleukin⁃6(IL⁃6),interleukin⁃1β(IL⁃1β),total cholesterol(TC),triglycerides(TG),low⁃density lipoprotein(LDL),and high⁃density lipoprotein(HDL).Real time fluorescence quantitative PCR assay was used to detect the levels of miR⁃146a and miR⁃155 in peripheral blood.The univariate a⁃nalysis of factors affecting the occurrence of coronary heart disease in patients with severe periodontitis,and logistic regression analysis of the influencing factors.The receiver operating curve(ROC)was used to analyze the value of miR⁃146a and miR⁃155 levels in pre⁃dicting the occurrence of coronary heart disease in patients with severe periodontitis.Results:The levels of miR⁃146a in group A were higher than those of the control group and group B respectively(P<0.05).The miR⁃155 levels in group A were lower than those in the control group and group B respectively(P<0.05).The body mass index,serum CRP,and miR⁃146a levels of patients in group A were higher than those in group B(P<0.05),while the education years and miR⁃155 levels of patients in group A were lower than those in group B(P<0.05).Logistic multivariate regression analysis showed that the average level of miR⁃146a was an independent risk factor for the occurrence of coronary heart disease in patients with severe periodontitis,but the level of miR⁃155 was a protective factor(P<0.05).ROC analysis showed that the optimal
